Job Description: Department: Last Revised: Salary Level: Enliven Team Lead Family and Youth Issues 6/15 6 Definition The Enliven Team Lead, is a Licensed Clinical Social Worker, working with clients 15 to 25 years of age who are experiencing, or in early stages of their first episode of a psychotic disorder. The Enliven Team Lead works collaboratively with the client and the Enliven team to assess, plan, develop, coordinate and provide treatment, rehabilitation and support. This position will report to the ENLIVEN Statewide Coordinator. Supervision Direct supervision of Enliven cabinet team members; indirect supervision of Enliven State employees Duties Cultivate referring networks including multilevel outreach to providers, clients, and families. Responds to enquires for services and completes client screenings and consultations. Coordinates clinical services among Enliven team members. Facilitates weekly team meetings and provides ongoing clinical supervision. Submits monthly reports regarding program activities to the Enliven Statewide Coordinator and funding sources as needed. Implements and evaluates measurable outcomes for the program. Manages and evaluates the staff in the program May be required to transport clients Provides individual and group psycho-education to families Engages youth and establishes trusting, collaborative relationships directed toward the goal of engaging the client in services. Documents, assesses, plans, and summarizes interventions and issues in timely and accurate manner. Maintains high quality records. Enters, updates, and monitors information in client database. Reads, understands and follows Enliven Policies and Procedures. Achieves an understanding of agency mission, strategies and objectives. Contributes to the success of these areas. Assists in outreach and recruitment activities Performs other duties as assigned. Entry Level Qualifications Knowledge of Psychosis and recovery process Community agencies and resources, as well as an understanding of the problems facing youth and families. Family and youth relationship issues. Sound decision-making, crisis management techniques, and be a self-motivated team player. Computer applications and the desire to learn new programs. Ability to Identify primary psychosis and perform differential diagnosis Keep appointments punctually, be organized and demonstrate follow through. Express ideas clearly and concisely both orally and in writing. Work with adults and youth of diverse backgrounds and beliefs. Problem solve in stressful situations. Work as a member of a multidisciplinary team. Special Requirements Licensed Clinical Social Worker with experience diagnosing and working with psychosis and related disorders Supervision experience. Bilingual preferred. Able to pass complete background check. Must have reliable transportation to get to and from work Must posses a valid Nevada Driver’s license; and proof of insurance if utilizing personal vehicle for company business. Willing to obtain needed medical screens and immunizations such at TB test, Hepatitis, etc. Able to lift and carry 10 lbs. Able to walk up and down stairs. License or Certificate Most possess licensure as a Clinical Social Worker through the state of Nevada Board of Examiners for Social Workers.
