BY CLICKING OR SIGNING, YOU ACKNOWLEDGE THAT YOU UNDERSTAND
AND ACCEPT THE TERMS, CONDITIONS AND POLICIES STATED BELOW***

Signing this form acknowledges that the applicant and their parents understand that dancing is a physical activity and any
injuries of any type or sort is not the responsibility of Simply Divine Academy of Dance, its employees, assigns, individual
contractors, and its owners from any type or nature whatsoever. This release of liability applies on, in or surrounding
premises of Simply Divine Academy of Dance. Also applies to family members or visitors when they bring students to
dance class. It also complies with all rules and regulations associated with Simply Divine. In the event there is an incident,
your signature allows Simply Divine to seek appropriate medical treatment or attention on the behalf or the child as may be
required by the circumstances, including but not limited to, medical doctor and/or hospital visits. This authority ends at the
termination of dance lessons.
I understand that the monthly tuition is due by the tenth of each month. After the tenth, of the month I understand that a
$30.00 late fee will be applied each month there is a balance
I accept the $25.00 NSF Fee
In the event of our child dropping or changing dance classes, I will complete the special form provided at the front desk to
notify the studio of these changes. I understand that one month’s written notice is mandatory and we are responsible for
that month’s tuition. Messages by phone or email are not accepted
I understand that our Tuition, Costume Fee, Printer Fee, Recital Fee and Video Fee is ‘NON REFUNDABLE’
I understand that wish ads are a separate fee aside to the printer fee
I understand that a successful dancer is dependant upon attendance to dance classes
I will be prompt to dance classes
