The Clinical Nurse Specialist provides specialist advice to healthcare professionals, other agencies, carers, clients and relatives for a specific field of practice. 3 ORGANISATIONAL POSITION Head of Nursing Lead Gastroenterology Clinical Nurse Specialist Nurse Endoscopist/ Clinical Nurse Specialist (this post) General Conditions 05.08 4 SCOPE AND RANGE To provide a high quality, safe and supportive environment in order to care for patients, clients, carers and relatives within Gastroenterology services particularly Endoscopy and including patients with specific Gastrointestinal disease to meet identified care needs. NHS Tayside covers some 7,558 square kilometres with a population of approximately 385,500. The Gastroenterology Nursing Service provides inpatient care over two sites, at Ninewells Hospital Dundee and Perth Royal Infirmary whilst offering a number of outpatient clinics including Stracathro Hospital. Endoscopy services are provided across all three sites at Ninewells, Stracathro and PRI. The Gastroenterology Clinical Nurse Specialist provides a wide range of specialist care and services for this particular client group in a variety of clinical settings throughout Tayside in response to service demand. In collaboration with the multi-disciplinary team the Gastroenterology Clinical Nurse Specialist is responsible for the diagnosis, treatment and management of adults with gastro-intestinal diseases and symptom. The Gastroenterology Clinical Nurse Specialist plays a pivotal role within the multi-disciplinary team working autonomously, co-ordinating the patient’s journey from point of referral, treatment, through to discharge and follow up care. This involves initiating and establishing nurse led clinics. The Gastroenterology Clinical Nurse Specialist works collaboratively with the multi professional team and works autonomously to assess, diagnose and recommend treatment to facilitate achievement of waiting list targets. This involves clinical history taking and physical examination Environment of Care: This post is hospital based, working within endoscopy units and outpatient departments across the 3 sites of Ninewells Hospital, Perth Royal Infirmary and Stracathro Hospital in response to service demand to promote cross cover across the directorate and to provide cover for colleagues as required. To include: 5 Liaises with national and international centres and agencies. Provide specialist advice to health care workers and the general public Budgetary Responsibility to ensure specialist departmental supplies and patient products are ordered and used economically and appropriately Supervisory responsibility for junior endoscopy/Gastroenterology Nurse Specialists MAIN DUTIES/RESPONSIBILITIES 1. Provide a specialist and advanced nursing contribution to the clinical management of a defined caseload of patients within Gastroenterology, working autonomously and in collaboration with multidisciplinary professionals in order to improve the care of patients with Gastrointestinal disease and in particular those undergoing endoscopic procedures. 2. Take a systematic and comprehensive patient history to make a holistic assessment of patient’s suitability to undergo endoscopic procedure on the day of appointment, taking into account current medication and physical well being i.e. anticoagulation therapy and potential need for a biopsy. Promote patient participation in decision making and obtain informed consent for examination to be performed ensuring that the nature, consequence and risks of the treatment are understood by the patient. 4. Assess anxiety levels of patient (having endoscopic procedures) on admission and plan coping strategies with patient, including the use of intravenous sedation when necessary. Administer intravenous sedation independently, within agreed local policy, and also administer intravenous reversal agents if required based on clinical judgement. 5. Perform Diagnostic and Therapeutic Sigmoidoscopy and Colonoscopy, working autonomously to assess, plan, deliver and evaluate care. 6. Perform biopsy of pathology encountered in order to establish a diagnosis. Follow up histology, interpret findings, make clinical judgements involving a range of facts that require analysis and comparison of a range of options to plan and initiate follow up management of patient towards appropriate treatment or specialist referral, without supervision, taking responsibility for assigning diagnosis. 7. Assess the health education needs of Gastroenterology patients attending for examination initiating health promotion and providing patients with written information as necessary on diagnosis. 8. Provide support for patients with cancer at initial diagnosis, providing information and emotional support where appropriate. 9. Produce computer generated reports and letters, utilising the Endoscopy Management System reporting tool effectively to produce accurate reports of examination findings to disseminate results to medical notes and referring Consultant or General Practitioners. 10. Provide a nurse-led out patient service for patients with gastroenterological disorders, ensuring accurate clinical history taking. Provide information, education and emotional support as required, promoting the concept of patient self-care, and ensuring appropriate investigations and referral to other members of the multidisciplinary team are requested, working within agreed protocols of practice, therefore improving the quality of life and compliance with treatment of patients within defined area of practice. 11. Review results of outpatient investigations i.e. blood results, DEXA scan reports etc and discuss any abnormalities with Consultant Gastroenterologist in order to effect appropriate clinical management. 12. Providing a specialist advanced nursing contribution to support, inform and educate defined group of patients during inpatient treatment periods. 13. Co-ordination of staging investigations and referral to other members of the multi-disciplinary team in order to manage and co-ordinate the patients journey from point of referral through to discharge and follow-up, to provide seamless care. 14. Daily planning and organisation of workload that frequently requires adjustment due to service demands. 15. Frequently provide highly specialised advice in relation to patient care and management. Accountable for direct delivery of a clinical service as agreed with local Division policy (e.g. sedation policy, waiting list initiatives). 16. Involved in the Development of policies and protocols for service changes within our own area, but which may impact beyond your own department. 17. General Conditions of Service POST REF NO: D/PS/208/14 JOB TITLE/GRADE: Nurse Endoscopist, Band 7 LOCATION: Endoscopy Unit, Ninewells Hospital Conditions of Service Remuneration Hours of Duty Superannuation The terms and conditions of service for this post are those determined by the NHS Staff Council. The current salary scale for the post is £31,072 to £40,964 per annum (pro rata for part-time staff). Placing on the scale on appointment is normally at the minimum but may be higher subject to verification of previous relevant service. Salary is paid monthly by Bank Credit Transfer. The hours of the post are 37.5 per week. . Start and finish times will be determined by the needs of the service. Membership of the NHS Superannuation Scheme is not compulsory but is open to all staff between the ages of 16 and 70 (65 in some instances).
