If you need medical attention outside surgery hours, please contact Shropdoc on 08444 06 8888. When you ring Shropdoc your details will be taken by a call handler. A Nurse or Doctor will then phone back. All calls are recorded. Your problem may be dealt with over the phone or you may be offered an appointment with a Doctor, or a Doctor may visit you. If you consider a problem to be life threatening you should dial 999 Appointments We see patients by appointment only and these can be booked up to one month ahead. We also offer patients a same day late morning appointment if you contact the Surgery before 11.00 am. Please phone 01939 260 210 to make an appointment. We offer 10 minute appointments with all of the Doctors and try to avoid delays where possible. Unfortunately we do sometimes run late for various reasons including patients having complex problems or an unforeseen emergency case. We would appreciate your co-operation by arriving on time and consider booking a double or additional appointment if you have several problems to discuss. If you need to cancel an appointment please try to give as much notice as possible. Home Visits Home visits can be arranged for those who are housebound due to illness. Most problems can be dealt with sooner and more efficiently if you attend the surgery. If you require a non-urgent visit please contact the Surgery before 11am. Telephone Advice If you require non-urgent phone advice, please contact the Surgery on 01939 260 210 and a Doctor will return your call later the same day. You do not require a Doctor's sickness certificate for any illness lasting seven days or less. Your employer may however require you to complete a selfcertification form (SC2) which is available from your employer or on the HMRC website www.hmrc.gov.uk/forms/sc2.pdf Evidence that you are sick If you are sick for more than seven days, your employer can ask you to give them some form of medical evidence to support payment of SSP (statutory sick pay). It is up to your employer to decide whether you are incapable of work. A medical certificate, now called a 'Statement of Fitness for Work’ (see below) from your Doctor is strong evidence that you are sick and would normally be accepted, unless there is evidence to prove otherwise. You could also provide evidence from someone who is not a medical practitioner, e.g. a Dentist. Your employer will decide whether or not this evidence is acceptable. If your employer has any doubts, they may still ask for a medical certificate from your GP. Statement of Fitness for Work - ’Fit Note' The 'Fit Note' was introduced on 6 April 2010. With your employer's support, the note will help you return to work sooner by providing more information about the effects of your illness or injury. In addition you can now order repeat prescriptions on-line. If you are interested in the on-line ordering please see our reception staff to register for this service. Our repeat prescription phone line 01939 261403 is staffed between 9.00am to 12.30pm and from 2.00pm to 5.00pm on weekdays. Outside these hours an answer phone is available for you to leave a prescription request. Please leave your name, date of birth, contact phone number and a list of the medications you require. Repeat prescriptions may be collected after 2.00pm the following working day. Occasionally there may be a delay in obtaining a particular medication. If you live in the Bomere Heath area we also provide a weekly medication delivery service to the village hall on Friday morning at 11.30am. Please inform our receptionist when ordering your repeat prescriptions if you wish them to be delivered here. Dispensing We are fortunate to have a Dispensary at the surgery and can dispense medication to most of our patients. Medicines prescribed during a consultation can usually be collected straight away. We are only allowed to dispense medicines if you pay the full prescription charge or give proof of exemption from charges. The prescription charge is set by the government, not the practice. Prescriptions Charges and Exemptions Extensive exemption and remission arrangements protect those likely to have difficulty in paying charges (NHS prescription and dental charges, optical and hospital travel costs). The NHS prescription charge is a flat-rate amount which successive Governments have thought it reasonable to charge for those who can afford to pay for their medicines. Prescription Prepayment Certificates (PPCs) offer real savings for people who need extensive medication. NHS charges Prescription costs as of April 2015 The current prescription charge is £8.20 A three monthly PPC is £29.10. This saves you money if you need four or more items in three months. A 12 month certificate is £104.00 and saves money if 15 or more items are needed in 12 months. PPCs are available by 10 monthly direct debit instalment payments. We do not allow our receptionists to disclose results over the telephone. If you wish to discuss the results of any tests please book an appointment or telephone consultation with the doctor. Medical Examinations We offer private medical examinations by appointment. These are not covered by the NHS and therefore a charge is payable. Registration New Patient Registration We accept new patients who live within our practice area. As it often takes some time for records to be forwarded from your former Practice all newly registered patients who are taking regular medications are offered a consultation with a doctor. Medical treatment is available from the date of registration. Please contact reception for further information. Confidentiality The Practice complies with Data Protection and Access to Medical Records legislation. Identifiable information about you will be shared with others in the following circumstances: To provide further medical treatment for you e.g. from district nurses and hospital services. To help you get other services e.g. from the social work department. This requires your consent. When we have a duty to others e.g. in child protection cases anonymised patient information will also be used at local and national level to help the Health Board and Government plan services e.g. for diabetic care. If you do not wish anonymous information about you to be used in such a way, please let us know. Reception and administration staff require access to your medical records in order to do their jobs. These members of staff are bound by the same rules of confidentiality as the medical staff. For this reason the Practice supports the NHS policy of Zero Tolerance. Anyone attending the Surgery who abuses GP’s, Practice Nurses, staff or other patients be it verbally, physically or in any threatening manner will risk removal from the Practice list. Prescott Surgery Complaints Procedure At Prescott Surgery we offer a complaints procedure to deal with any problems or complaints you may have. Our aim is to resolve within the Practice any problems that may have arisen, to deal with as quickly as possible, and to ensure that our service to you is of the highest standard. If you have a problem or a complaint, please speak to our Practice Manager Nicolas Storey, or to one of our senior Receptionists. If you prefer, or if your complaint is complicated, please send it to us in writing. Complaints must be made within 6 months of the event, giving rise to the complaint, or within 6 months of becoming aware that there is a cause for complaint. We must respect patient confidentiality if the complaint involves someone other than yourself; you must have the person’s permission. We have a consent form for this purpose. We will acknowledge your complaint in writing or by telephone within 3 working days. Your complaint will be thoroughly investigated by a senior member of staff who is not the subject of the complaint. We will tell you who is dealing with the complaint when we acknowledge your complaint. When he/she has gathered as much background information as possible, we shall let you know what we have found out. If you are happy with our explanation we will not take it any further. If you are not happy however, or if there are more issues to be looked at, we may invite you to a meeting at which we can discuss your complaint. You are welcome to bring a friend or relative with you if you find it helpful. This will be an informal meeting which aims to explore the issues relevant to your complaint. British Thoracic Society www.brit-thoracic.org.uk Travel Information PLEASE BE AWARE THAT WE ONLY DO BASIC TRAVEL The following information is only for more complicated travel which we do not cover If you require any vaccinations relating to foreign travel you need to make an appointment with the practice nurse to discuss your travel arrangements. This will include which countries and areas within countries that you are visiting to determine what vaccinations are required. There is further information about countries and vaccinations required on the links below It is important to make this initial appointment as early as possible - at least 6 weeks before you travel - as a second appointment will be required with the practice nurse to actually receive the vaccinations. These vaccines have to be ordered as they are not a stock vaccine. Your second appointment needs to be at least 2 weeks before you travel to allow the vaccines to work. Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS. Community Midwives The Midwife sees antenatal patients at the Surgery on Friday mornings; she also performs antenatal checks at patient’s homes, and visits new babies and their mothers in the first 10 days. She can be contacted via the hospital (01743 261000 – ask for – “Community Midwives”). Community Nurses District Nurses provide care for house bound and terminally ill. They can be contacted on 01939 261373 (be prepared to leave a message on their answering machine). Health Visitors The Health Visitor will deal with immunisations, child health problems and perform developmental checks on pre-school children. Counsellors A Counsellor holds weekly sessions at the Surgery. Please see a GP if you wish to be referred to the Counsellor. Attached Staff Chiropodists visit the Surgery on Mondays and some Tuesdays. They offer general foot care and assessment especially in older patients and diabetics.
