I have requested a Heaven’s Best franchise technician to examine carpet or upholstery that has been damaged with a color dye. I acknowledge
that the carpet, or upholstery in its stained condition, is of lessened value to me. It has also been explained to me by a Heaven’s Best
technician that Red Dye Remover in most cases is an effective method of removing color stains caused by most food grade dyes.
There are variations in carpet and upholstery fabric and dyestuff composition and that there are instances where color stains might not be
removed and where discoloration and/or bleaching of the carpet or upholstery may occur. I also understand that it is impossible to test for or
predetermine the result of the Red Dye Remover treatment.
With full knowledge of the above, I have requested the Heaven’s Best franchise technician to treat my carpet or upholstery with Red Dye
Remover in an attempt to remove the color stain. As there are variables beyond the control of Heaven’s Best and their technicians, I
understand that the Heaven’s Best franchise and M-CO Inc. disclaims any liability for damage caused to the carpet or upholstery as a result of
the Red Dye removal treatment. I agree to hold the Heaven’s Best franchise and M-CO Inc. harmless for any damage caused to my carpet or
upholstery as a result of the Red Dye stain removal treatment.

PET URINE TREATMENT
WAIVER AND DISCLAIMER
I fully understand that my carpet or upholstery is currently damaged due to urine deposits. Because urine is a complex mixture of ingredients,
I understand that undesirable results may occur in the form of slight carpet shrinkage, wicking, browning, yellowing and /or discoloration. I
hereby authorize the Heaven’s Best franchise technician to proceed with the Pet Urine Treatment. I agree to the stated price for the services
rendered and understand that there is no guarantee implied or expressed that any or all urine odors, or stains will be removed.

UPHOLSTERY CLEANING
WAIVER AND DISCLAIMER
Backing, lining, and other materials and processes used in the manufacture of upholstery may produce adverse results in the surface material
when cleaned. There is no way of determining when such results will occur. The fabric may have previously experienced deterioration due to
such factors as fading, shrinkage, dry-rotting, watermarking, browning or loss of tensile strength, prior use of cleaning agents and improper
cleaning by other cleaners. Therefore, we as cleaners will not be responsible for any of the conditions listed above.

I acknowledge that the Heaven’s Best franchise technician has explained to me the Hardwood Floor Cleaning process. He has informed me
that this process can leave the floor slippery when walked upon. Caution should be taken when walking on the floor. If any accident should
occur, I do not hold Heaven’s Best or the technician responsible.
I acknowledge that I have read the waiver(s) and disclaimer(s) next to my initials above and hereby authorize the Heaven’s Best franchise
technician to proceed.
