Allergic evaluation can be a very valuable experience. Our goal is to identify the etiologic agent(s) that may be causing the patient’s problem and to eliminate exposure. This is done by a detailed history, followed by a physical examination, then appropriate skin testing. Frequently, patients are allergic to more than one allergen. They may be sensitive to allergens such as dust, pollens, or molds that cannot be completely eliminated from their environment. Skin testing is a helpful aid to diagnose the specific causes of allergies. Other laboratory studies may be necessary in evaluating the problems of the patient. Appointments: Please call as far in advance as possible to cancel any appointments so that we may use the time for another patient. Emergency treatment for established patients: If you are having difficulty during the day, please call our office for an appointment. The office is open Monday, Tuesday, Wednesday, and Friday (closed Thursday) from 9:00 a.m. to 5:00 p.m. If you are having difficulty when the office is closed, please call the Physicians’ Exchange at (309) 691-5200, or report to the nearest emergency room facility. Satellite offices: We have satellite offices in Pekin, Macomb, and Princeton. If any of these satellite offices are closer to where you live, please call (309) 691-5200 for an appointment. Satellite office hours are as follows: Pekin (2nd and 4th Thursdays of each month). Macomb (1st and 3rd Thursdays of each month). Princeton (2nd and 4th Tuesdays of each month). Please note, all dates are subject to change. Family Physicians: Treatment in our office is limited to the care of the allergic condition; therefore, it is essential that you have a family physician or pediatrician for the general care of the patient. Billing and insurance information: We participate in most major health plans in the Peoria area, including Caterpillar/United Healthcare, OSF, BC/BS, Methodist First Choice, Mutual Medical, Humana, and HealthLink. If you have any questions regarding your coverage, it is recommended that you check with your insurance agent PRIOR to your appointment as to your deductible and/or coverage. Insurance: As a courtesy, we will submit all claims to your primary insurance; however, if you wish for us to bill your secondary insurance, a copy of the primary Explanation of Benefits must be mailed to our office with a note attached requesting our office bill the services to your secondary insurance. Medicare: Since we are a non-participating provider with Medicare, all Medicare checks will be issued and mailed to the patient. All co-pays and/or deductibles are due at the time services are rendered. If there is a problem in making payments, please call and make arrangements with the office manager. In case of divorce, with the patient being a child, whoever brings the child in for treatment is responsible for presenting payment at the time of service, regardless of divorce decrees Miscellaneous: 1) Should there be a change in your address, telephone number, marital status, insurance company, etc please advise someone in the front office. 2) Should your family find themselves in a situation which causes your medical expenses to be a difficulty, please discuss the problem with the doctor or the office manager. 4) We charge $10 for returned checks due to non-sufficient funds.
