This appendix provides examples of an acceptable format for records of medicine usage and purchase. All records marked * are legal requirements. Others are required by the scheme as best practice. Both the Pig Veterinary Society (PVS) and the National Office for Animal Health (NOAH) also publish templates for medicine records. Although each has small differences, used properly, both meet the requirements of the assurance standards.
