Toledo Hospital Family Medicine Residency Flower Hospital Family Medicine Residency PGY-1 Pharmacy Residency Toledo Hospital Family Medicine residency: There has been a clinical pharmacist on faculty at the Family Medicine residency at Toledo Hospital since the late 70’s. Originally a shared position with the pharmacy department and University of Toledo College of Pharmacy, the position was moved fully within the residency in 1995. Flower Hospital Family Medicine residency: Like the residency at Toledo Hospital, the Family Medicine residency at Flower Hospital has been training Family Physicians since the mid 1970’s. In 1991, in response to the faculty’s request, a pharmacist rounded one day per week and gave a monthly lecture. In 1992, a new pharmacist faculty position was created fully within the Family Medicine residency. The Pharmacy residency: The pharmacy residency was started in 2002 as a joint effort between the Family Medicine clinical pharmacists. Initial funding came from a grant from the Area Office on Aging for pharmacists to provide education about medications to the seniors at eight local senior centers. The original residency was a Primary Care Specialty Residency. With the change in nomenclature from ASHP, we became a PGY-2 Ambulatory Care residency. We earned ASHP accreditation beginning in 2004. Due to a limited number of PGY-1 graduates interested in a PGY-2 Ambulatory Care residency, we elected to change our status to a PGY-1 residency in 2007. ASHP surveyed our residency in 2008 and awarded Preliminary Accreditation status. We anticipate full accreditation status when the ASHP accreditation committee meets in March, 2009. Our Residents & Graduates: Year Resident / Graduate 2002-3 Amber (Zabawa) Zuber Amber’s first position after residency was with the Cleveland Clinic in ambulatory care and anticoagulation management. Amber currently provides MTM and other services within the prescription benefits department of Paramount Insurance. 2004-5 Megan Yousif Megan’s first position was as an Ambulatory Care clinical pharmacist with the Shands Hospital in Jacksonville, Florida. Megan has since relocated closer to family and works at St. John’s Hospital in Michigan. Year 2005-6 Resident / Graduate Megan Kaun and Lori Ernsthausen Megan Kaun is currently the Director of Advance Pharmacy Practice Experiences with the University of Toledo College of Pharmacy. She also teaches in the pharmacotherapeutics course and maintains a clinical practice at the Center for Health Services at Toledo Hospital. Lori Ernsthausen is an Assistant Professor of Pharmacy Practice at the School of Pharmacy, University of Findlay. 2006-7 Julie Skerl and Katherine Owens Julie working at a small hospital near Akron as a staff clinical pharmacist. Katie recently relocated to North Carolina and is working in community pharmacy (CVS). She recently accepted a part time position at Wake Heart & Vascular associates where she sees patients in both warfarin and lipid clinics. 2007-8 Laura Perry and Aaron Lengel Laura Perry is an Assistant Professor of Pharmacy Practice at the School of Pharmacy, University of Findlay. Aaron Lengel is a visiting professor at the University of Toledo, College of Pharmacy. He is developing a clinical practice at the Cordelia Martin clinic and he is involved with the UTCP / Kroger joint Community Pharmacy residency. 2008-9 Current residents Julie Bockert and Jon Lindau
