JOB DESCRIPTION
INTENSIVE CARE UNIT
THE DISTRICT AND HOSPITAL
The Hillingdon Hospital is a busy modern 600 bedded district general hospital dealing with
the majority of medical and surgical specialities and has Trust status. Our patients come
from the London Borough of Hillingdon (population about 250,000) and from Heathrow
Airport, lending an international flavour to admissions.
There are strong links with several medical schools and students from Imperial College
Medical School do part of their training here. The hospital hosts the MRCP exam each year.
THE WORK OF THE DEPARTMENT
The 9 bedded ICU/HDU cares for adult medical and surgical patients.
The ICU and HDU share nursing and medical staff and are used flexibly depending on the
patients dependency, such that ICU/HDU bed ratio may vary.
There are over 360 admissions per year and 60% of the patients are ventilated.

The unit is well equipped with high tech monitoring and facilities for advanced organ support
and is committed to the latest treatment techniques.
Resident Senior House Officers benefit from close consultant supervision and bedside
teaching. There are 2 daily ITU consultant rounds and a Microbiology/Pharmacology ward
round at midday. Senior House Officers will also have the opportunity to be trained in the
insertion of lines for vascular access, percutaneous tracheostomy, bronchoscopy and insertion
of chest drains when required. There is also have the opportunity to learn airway
management skills if desired in theatres.

DUTIES OF POST:
This is a four month post with a 56 hour week - full shift system with consultant supervision
during daytime hours.
 The postholder will plan and institute the general supportive care of critically ill patients
under the supervision of the ICU Consultant. This will include clinical examination and
documentation, the performance of technical procedures, prescription of drugs and
requests for investigations.
 Also the postholder will institute specific medical care prescribed by the referring clinicians
and will be present at all discussions concerning the care of patients.
 The postholder will control admissions to and discharges from the ICU, in consultation with
the referring clinicians, the ICU consultant and the Nursing staff.
 The postholder will attend ward rounds and present the patients.
 The postholder will attend tutorials and audit meetings.

 As the co-ordinator of patient care, the ICU postholder will maintain good communications
with all disciplines within the ICU, with outside departments and with other professionals
involved in continuing patient care. He/she will also ensure that accurate, timely and
consistent information is given to relatives and help to provide advice, support and
counselling to them.
 The postholder will keep accurate records and participate in data collection for audit.
 Other duties include occasional management of emergencies in the A&E department or the
wards in collaboration with the duty anaesthetist, provided he/she is available to return to
the ICU.
 Inter hospital transfer of patients. (as appropriate)
Out of hours the Critical Care Physician on duty for Intensive Care is supported by the on-call
anaesthetic SHO and SpR and by the “on-call” anaesthetic or ICU consultant.

On Call Commitment:
Only 1 ICU SHO grade is allowed annual leave at any one time. There is no night time on call
associated with this post; although long day shifts (8am-8pm) do include the weekends. A
registrar is also on call for the unit during these shifts.
Emergencies and Unforeseen Circumstances:
The junior doctor accepts that he/she will also perform duties in occasional emergencies and
unforeseen circumstances at the request of the appropriate consultant, in consultation where
practicable, with colleagues, both senior and junior. It has been agreed between the
professions and the Department of Health that whilst juniors accept that they will perform such
duties, the Secretary of State stresses that additional commitments arising under this
subsection are exceptional and in particular that juniors should not be required to undertake
work of this kind for prolonged periods or on a regular basis.
LEARNING METHODS AND OPPORTUNITIES:






Weekly tutorials in conjunction with the anaesthetic department.
Specific ICU based tutorial sessions.
Twice daily ward rounds.
Monthly Audit meetings.
Release where possible to attend the Pan ICB ICU training days (1 per month)
ALS training
All trainees are encouraged to attend day release courses or intensive courses in preparation
for examinations in accordance with the Regional Guidelines and the British Postgraduate
Medical Federation Doctors' Study Guide.
Time for private study before examinations may be possible within study leave entitlement.
There is a well equipped medical library and research facilities. We have an active
postgraduate teaching programme in a modern Postgraduate and Research Centre on the
hospital site. Dr R Kantor, Consultant Radiologist, is the current Clinical.
The unit is recognised for training at SHO level by the Intercollegiate Board of Intensive Care
Medicine.

CONDITIONS OF EMPLOYMENT
Applicants should be in possession of the Primary FRCA, Part I MRCP or Part I FRCS.

CONDITIONS OF SERVICE
This post is covered by the Terms and Conditions of Service of the Hospital Medical and
Dental Staff (England and Wales), which should be read in conjunction with the General
Whitley Council Conditions of Service.

The post is compliant with New Deal and European Working Time Directive

It is a whole time appointment.

Salary scale:
£30,002-£47,175 per annum pro rata
London weighting:
£2,162 per annum pro rata
c)
Appointment to this post is subject to satisfactory medical clearance.
d)
Accommodation: Limited furnished bed/sitting rooms may be available on-site for full
residence. This is not a recognised resident post but you are required to be resident
when on-call. On-call accommodation is provided.
As from 1 April 2002 you will be required to complete an Enhanced Disclosure Application
Form, which has been requested by the Criminal Records Bureau (i.e. Police check) for all
successful applicants.
OTHER FACILITIES
There is a small parade of shops almost opposite the hospital. Uxbridge is the nearest major
shopping centre, about 3 miles away.
Bus services U4 and U5 from Uxbridge to Hayes and Heathrow Airport respectively. Both
services stop at the hospital.
The nearest underground station is Uxbridge. Metropolitan and Piccadilly lines both run from
Uxbridge. West Drayton is the nearest British Rail station. Western Region connection to
Paddington.
There is easy access to the M4, M40 and M25 motorways.
