later than 2.30pm on Thursday 22nd January 2015. This proposal is subject to the approval of the Department of
Philosophy. Please note that, for applicable modules, if you do not obtain the required signatures on the
attached Essay Titles sheet and/or you do not submit these forms by the stated deadline, the default position of
examination by invigilated exam will not be altered on your student record and you will be required to sit the
appropriate invigilated exam.

Apart from PH201 HMP and PH304 Textual Studies, no row in the table should have more than 1 assessment method
ticked. If you have chosen to be assessed by essay, please also state in the end column whether you are using one of the
pre-approved titles published by the module tutor. If you are doing a Dissertation, Independent Project or using
your own essay title please make sure you seek signed approval from your supervisor or/and module leader(s)
and fill in the last page of this form with your title(s).

DECLARATION concerning PLAGIARISM and SUBMISSION DEADLINE
1. In making this application to submit assessed work in lieu of an invigilated examination I confirm that I am
aware of the University’s Regulations governing Cheating in a University Test (Regulation 11) and of the
further guidance issued by the Department of Philosophy concerning plagiarism and proper academic practice.
I recognise that the assessed work that I shall submit must be in accordance with this Regulation and this
guidance. I am aware of the penalties that may be imposed in the event that the written work submitted by me
is not wholly my own.
2. I undertake not to submit (without written permission) work that has previously been submitted either in
whole or in part for another qualification at this or another institution.
3. I recognize that infringement of either 1 or 2 may make me liable to proceeding under Regulation 11(B).
4. I acknowledge that I have read and understood the regulations issued by the Department of Philosophy
concerning the deadline by which assessed work must be submitted. I am aware that if the assessed work is
not submitted to the Philosophy Office by 12pm (noon) on Tuesday 28th April 2015 (non-finalists and
finalists have the same deadline), then it will attract a penalty of 5% per working day (or part of a working
day) if it is submitted late.
