From the Utah State Division of Aging INSTRUCTIONS This document is a flip chart. Gather 12 different colored sheets of paper to make 13 copies. Copy each page a different color except the last two pages, which should be the same and in an off white color. The last two pages are used to draw individual evacuation plans for the facility - could be one or two or more areas. Cut each page horizontally directly under the bold line. Line up all pages with the top page - there should be a gradation of topics appearing at the end of each page. Staple the top on both sides. Distribute to the senior centers. Center Directors should hold an in-service, review the entire plan, make the evacuation drawing/s and post this on their bulletin board for everyone to see. 4/02 1 From the Utah State Division of Aging EMERGENCY PLAN INTRODUCTION PURPOSE: The purpose of the Emergency Preparedness plan for ____________________ Senior Center is to provide information and direction for all persons with emergency response tasks. This includes management and support staff and center clientele. AUTHORITIES: Area Agency on Aging directives and directives from the State Division of Aging and Adult Services. CHEMICAL SPILLS Warning: City Policy Chief or County Sheriff. Center Support Staff: Evacuation – same as flood/mudflow (walking to evacuation site(s) as a last resort). Give warning and instructions to evacuate or remain in building. Coordinate with local authorities: Police _________________. Keep Area Agency on Aging appraised of situation: _______________. Center Support Staff: Account for all center clientele. Assemble everyone in _________ room. Close all doors and windows. Insure that no one leaves the Senior Citizens Center or building until directed to do so. Center Clientele: Stay indoors. Follow Center Support Staff instructions. POWER OUTAGE/OTHER UTILITY PROBLEMS Center Manager: Communicate with appropriate local utility company to determine extent and estimated length of outage. Communicate with Area Agency on Aging to relay problems. Obtain directive from City Police Chief or County Sheriff whether to dismiss Center clients and staff. If evacuation is necessary, follow evacuation procedures. Continue with your activities until directed to do otherwise. STORMS STORMS/BAD WEATHER: The nature of the problem will determine the exact course of action. Flood/Mud Flow: Follow outlined procedure. Tornado: Follow nuclear attack procedure. NOTE: In the event the road is closed and we continue to keep the Senior Citizens Center open, transportation arrangements will be altered to assure safe transportation of center clientele to their homes. Continue with your activities until told to do otherwise. CIVIL DISTURBANCE Warning: Warning will be given by City Police Chief or County Sheriff. If you see suspicious or threatening looking strangers around the Senior Citizens Center, notify Center support staff. EARTHQUAKE Warning: There will be no prior warning before the quake is felt. DURING THE QUACKING: ____________________________________ EVERYONE: INSIDE: STAY INSIDE. Immediately get under tables or in an inside doorway until quaking stops. Stay away from outside walls, lofts, stairways, and windows. OUTSIDE: STAY OUTSIDE. Stay in an open area away from buildings, chimneys, and power lines. AFTER THE QUAKING STOPS: _________________________________ Center Manager: Coordinate safe evacuation as in FIRE. Center Support Staff: Evacuate in orderly fashion and follow same procedure as in FIRE. Center Clientele: Follow directions of the Center Support Staff. FIRE Warning: The fire alarm will be activated by the first person to notice the fire. This person will also notify the Center Manager or Center Support Staff. Center Manager: Implement evacuation procedures. Coordinate with local authorities. Police: _______________________ Fire: _________________________ Inform Area Agency on Aging: _______________________ Follow up on Center Support Staff responsibilities. Center Support Staff: Shut off light. Close doors and windows. Take sign-up sheets to assembly area. 4/02 4 From the Utah State Division of Aging Lead Center clientele outside. Account for all Center clientele. Maintain order of Center clientele. Remain with Center clientele until all are either transported to their homes or allowed to return to building. Evacuation and assembly will follow the evacuation plan. Center Clientele: Follow directions of the Center Support Staff. Line up in orderly fashion and WALK out of building. In the winter try to get your coats, but don’t waste time looking for purses or other personal possessions. Remain in the assembly area until the Center Support Staff gives you other instructions. FLOOD/MUD FLOW Warning: Call to evacuate will be received from the City Police Chief or County Sheriff. Center Manager: Announce danger. Coordinate with local authorities. Police: __________________ Inform Area Agency on Aging: ___________________ Make decision with advisement of local authorities to do one of the following: Remain at Senior Citizens Center. Send or transport Center clientele home. Walk to evacuation sites; see “note” below. Coordinate the evacuation. Follow up on Center Support Staff responsibilities. NOTE: If the circumstances and time allow, Center clientele will be bused to the nearest evacuation site. Center Support Staff: Follow fire evacuation procedures to get Center clientele to assembly area. Follow exact evacuation routes. Account for all Center clientele present when the emergency occurred. Center Clientele: Follow fire evacuation procedures. Follow Center Support Staff instructions. NUCLEAR PROBLEM 4/02 5 From the Utah State Division of Aging Warning: Will be received from City Police Chief or County Sheriff. If at all possible, Center Manager, Center Support Staff and clientele will go home by their usual means. If staff and clientele are to remain at the Senior Citizens Centers, the following action will be taken: Center Manager: Sound the warning. Coordinate evacuation – if time permits it. Coordinate with Area Agency on Aging and local authorities: Area Agency on Aging: ________________ Police: ______________________________ Direct the nutrition program service in preparing and rationing food and water to Center Support Staff and clientele. Direct custodial personnel in providing safe drinking water and toilet facilities. Center Support Staff: Close all doors and windows. Center clientele on main floor will evacuate to lowest floor. Center clientele on top floor will evacuate to lowest floor. Instruct Center Clientele not to look at blast or out the windows and to take cover as you do in an earthquake until told to move. Maintain control of group and account for all staff and Center clientele. If and when building is evacuated after the blast, maintain order during period of release to go to individual homes. Center Clientele: Follow Center Support Staff instructions. Evacuate to the lowest floor. Do not look at blast. Curl up in a ball, covering your face, arms, and hands. If there is time, you will be allowed to return to your individual homes. BOMB THREAT Because a bomb threat may be received by anyone in the Senior Citizens Center, everyone should have specific directives for gaining full information from a caller, for relaying the threat to the proper authorities and for being appraised of evacuation and search of building procedures. RECEIVING A BOMB THREAT Instructions: Be calm and courteous. LISTEN! Do not interrupt the caller. Quietly attract the attention of someone nearby, indicating to them the nature of the call. Activate trap if telephone is so equipped. Complete this form when the caller hangs up. Does caller appear familiar with building by his description of bomb location? Write out the message in its entirety and any other comments on a separate sheet of paper and attach to this sheet. ACTION TO BE TAKEN AFTER RECEIVING THE BOMB THREAT: Notify the Center Manager or Center Support Staff. Talk to no one other than so instructed by the Center Manager or the Center Support Staff. Usual Procedures are as follows: (Exceptions will be dealt with on a case-by-case basis). CENTER MANAGER: Relay Bomb Threat to Police Department: _____________________. Code Words: Code words shall be utilized in an announcement to Center Support Staff for purposes of: Alerting the Center Support Staff of a bomb threat situation (with time permitting search of area) as opposed to a normal fire drill. Not alarming the Center clientele unnecessarily. Code words “Condition Amber”: 4/02 7 From the Utah State Division of Aging “Condition Amber” designates that sufficient information has been gathered to assume time is available for a search of the Senior Citizens Center for possible bomb(s) and evacuation f the building will be delayed or possibly cancelled. Code Words “CONDITION RED”: “Condition Red” designates that a bomb threat has been received but no information is available as to time and area of bomb explosion, or that a definite time for bomb explosion was received and it becomes imminent for immediate building evacuation of all personnel. Evacuation: Follow evacuation plan. Building Search Procedure: Search Plan – “Condition Amber” (no immediate evacuation): All personnel upon receipt of code words “Condition Amber” will search for unusual objects or packages. Do not remove or disturb. Report findings immediately to Center Manager. Police or bomb disposal squad will remove object. Center Support Staff – search center activity rooms. Custodian – search: Ground floor of Senior Citizens Center other than activity rooms and cafeteria. Boiler room. Basement. Center Manager – search: Second floor of Senior Citizen Center other than activity rooms. Restrooms. Library, workroom, health and lounge rooms. Kitchen/dining area, if nutrition support staff have gone for the day. Center Receptionist – search: Center Manager’s office. Center Support Staff office. Nutrition Support Staff – search kitchen/dining area. All personnel report findings (positive or negative) to Center receptionist. Search plan – “Condition Red”: None – Immediate evacuation. At discretion of Police Department, the following key personnel will be used to make search of the building: Center Manager. Center Support Staff. Custodian. Other designated volunteers 4/02 8
