Instructions for Completing Application for CECBEMS Organizational Accreditation Introduction This application and the requested accompanying documents will allow the Continuing Education Coordinating Board for Emergency Medical Services (CECBEMS) to assess the resources and infrastructure of an organization to determine whether or not it has appropriate institutional commitment to support a quality EMS continuing education program. Further, CECBEMS will assess the organization to determine whether or not it has an appropriate process in place for reviewing activities offered by cosponsoring organizations to ensure that these activities have undergone a sound educational process for planning, implementation and evaluation and that appropriate reports to CECBEMS Headquarters will be made and records maintained. The application asks for documentation that the organization will be able to meet all of the Standards and Requirements for Organizational Accreditation. For the purposes of this application program refers to the overall EMS continuing education effort of the organization; activity refers to an individual educational activity. Accredited organization refers to an organization that has been accredited by CECBEMS under the Standards and Requirements for Organizational Accreditation; cosponsoring organization refers to organizations that will submit activities for accreditation to CECBEMS accredited organizations; member organization refers to those organizations represented on the CECBEMS Board of Directors (National Association of Emergency Medical Technicians, the American College of Emergency Physicians, the National Association of Emergency Medical Services Physicians, the National Association of State Emergency Medical Services Directors, the National Council of State Emergency Medical Services Training Coordinators, and the National Registry of Emergency Medical Technicians, and the National Association of EMS Educators and the American College of Osteopathic Emergency Physicians). Activities offered by an accredited organization must undergo review of the accredited organization’s program committee just as activities submitted by a cosponsoring organization. Confidentiality No CECBEMS member, staff, reviewer or other individual affiliated with CECBEMS, its accredited organizations, or any of its member organizations may release to any person any materials or information submitted to or produced by CECBEMS, its members, staff, or reviewers in connection with a continuing education activity review conducted by CECBEMS without approval from CECBEMS and the express written consent of all parties to the review. This includes the identity and specific comments of reviewers acting on behalf of CECBEMS. All reviews and decisions are the product of CECBEMS and not of individual reviewers. Instructions for Completing Application for CECBEMS Organizational Accreditation 7/05 1 Instructions for Completing Application for CECBEMS Organizational Accreditation – 7/05 No CECBEMS member, staff, reviewer or other individual affiliated with CECBEMS, its accredited organizations, or any of its member organizations may release to any person copyrighted material received in connection with a CECBEMS Format IV application without approval from CECBEMS and the express written consent of the copyright holder. All CECBEMS reviewers must sign an agreement assuring that they will comply with these stipulations. For research purposes, data may be collected from materials submitted to CECBEMS. This data will not be released in any format in which any particular data can be linked to a specific organization. Maintenance of Accreditation CECBEMS reserves the right to perform random or for-cause site visits at its sole discretion to verify compliance with rules and procedures. In the event that the program is found in violation of its accreditation terms and conditions, the organization is subject to payment of all travel expenses and other expenses associated with the investigation and immediate termination of CECBEMS accreditation. If the organization is in compliance, all expenses will be paid by CECBEMS. CECBEMS may, at its sole discretion, require accredited organizations to verify state approval on an annual basis. Correspondence All correspondence about your application for CECBEMS accreditation should reference your organization’s four-digit CECBEMS ID number and prefix (assigned when you submit the Request for Application and be addressed as follows: CECBEMS c/o Liz Sibley 5111 Mill Run Road Dallas, Texas 75244 How to Submit the Materials Submit four copies of the material, each copy in a separate, loose-leaf binder with rings no larger than 2”. We recommend that you keep a fifth copy in your files to reference if CECBEMS calls with a question about your application. Tab1 A copy of your organization’s mission statement with the EMS continuing education mission highlighted and a written statement approved by the appropriate governing body that supports your organization’s EMS continuing education mission. Tab 2 Job description for person who will be directly responsible for ensuring that this organization’s and any cosponsoring organization’s education program will meet all CECBEMS accreditation requirements Tab 3 Example of each type of needs assessment used by the organization during the designated year Tab 4 Copy of the letter or other appropriate documentation of your organization’s recommendation or approval from your state’s EMS office, showing at a minimum that both the state EMS director and training coordinator find the program to be a reputable and reliable provider of quality EMS continuing education. Tab 5 Job description for this person who is your organization’s full-time educational program coordinator including EMS continuing education-specific responsibilities Tab 6 Copy of the EMS continuing education program budget for the reporting year you selected Tab 7 Copy of an inventory of equipment and supplies along with the cleaning and replacement policies Tab 8 Minutes from meeting(s) of program committee for one year or minutes from organizational meeting Tab 9 Copy of your organization’s agreement or contract with the medical director Tab 10 CVs of all members of the program committee and any rules or procedures concerning how the committee votes to approve activities 3 Instructions for Completing Application for CECBEMS Organizational Accreditation – 7/05 Tab 11 Minutes from all prior meetings of the program committee during the past year including meetings of a minimum of three members to approve activities submitted by cosponsoring organizations during the year Tab 12 Materials from one activity of each type in which you delineate the educational needs addressed by the activity, the learning objectives for the activity, the prerequisites for participants, and the expected learning outcomes. Tab 13 CVs for all instructional personnel for at least three activities. If possible, select an activity that includes lab assistants; CVs and, if appropriate, documentation of national registration or state certification of preceptors. Tab 14 Overall activity evaluation instrument and the participant evaluation instruments from at least three activities, a summary of the data collected by these instruments and an explanation of how you used the data to improve future activities Tab 15 Marketing materials for at least three activities offered during the year.
