The Sutton Wellbeing Hub opened their new wellbeing service on 1 July 2015 and is being delivered in the heart of the community in Sutton. The service is an integral part of the new Primary Care Mental Health service delivered by South West London St Georges Mental Health NHS Trust. Led by Imagine the Wellbeing Hub Partners consist of Age UK Sutton, Off the Record and Sutton Carers Centre. Between them the partners have experience and expertise in the provision of a wide range of supportive and socially inclusive services; linking people into community opportunities in Sutton and providing user led and recovery focused services. We are recruiting a further member to join the team who will be employed directly by Sutton Age UK as follows: Ref: 15/36 WELLBEING NAVIGATOR - OLDER PEOPLE - Based in London Borough of Sutton Full time including evening & Saturday am work (exact hours to be confirmed); Salary £24,500 a year The role of the navigator is to work with individuals who have a range of needs including social isolation, long term health conditions, anxiety and/or depression to access a wide range of services and community based resources. The successful postholder will be required to have awareness and be involved with engagement activities for older people including wellbeing support and strategies. The Navigators are currently based in the Jubilee Health Centre East in Wallington but will also work across the borough and be part of a multi-disciplinary team, ensuring a coordinated approach to support. A flexible working approach is required as well as team working, self management skills and outreach working. The successful postholder will have skills in assessing and developing person centred support plans, ensuring people have timely access to community or clinical provision that best meet their needs; have the ability to motivate, empower, increase choice and promote independence; network with existing and new providers and have experience in social inclusion, mental health (or people related) work. The scope, for creativity is excellent, you will develop bespoke activities to mentor individuals who may struggle to engage with community and vocational activities.
