SCOPE OF WORK
1.
GENERAL SCOPE
Contractor agrees to pick-up and legally/safely dispose of medical, biohazardous, sharps and pharmaceutical waste from a designated location
at Canyon Springs to a licensed/certified treatment facility. All removed
bio-hazardous waste shall be safely collected, handled, transported,
stored and disposed of by Contractor in full compliance with the California
Code of Regulations, Title 8 “General Industry Safety Order,” California
Health & Safety Code (CHSC) sections 117600 – 118360 (“Medical Waste
Management Act”), the Resource Conservation and Recovery Act of 1976
(RCRA), and all other related laws, regulations and ordinances governing
the environmentally responsible management of bio-hazardous and
pharmaceutical waste.

REQUIRED LICENSES/PERMITS
Contractor must have and maintain a Hazardous Waste Hauler’s permit,
issued by the Department of Toxic Substances Control.
Contractor shall furnish Canyon Springs a copy of their Hazardous Waste
Hauler’s Permit (4-digit number) from the Department of Toxic Substances
(DTSC).
Contractor shall provide Canyon Springs a copy of the Transfer Station
and Offsite Medical Waste Treatment Facility Permit, issued by the
Department of Health Services.
Contractor shall provide Canyon Springs a copy of Contractor’s business
license.

CONTRACTOR’S QUALIFICATIONS/REQUIREMENTS
Contractor must be in full compliance with, and have current knowledge
of, the California Highway Patrol regulations; the California State Fire
Marshall’s regulations; and the United States’ Department of
Transportation regulations. In addition, Contractor must comply with
Division 20, Chapter 6.5, Article 6 and 13 of the California Health and
Safety Code, and the Title 22, Division 4.5, Chapter 13, of the California
Code of Regulations, which are administered by DTSC.
Contractor must be in full compliance with, and have current knowledge
of, the California Code of Regulations, Title 8 “General Industry Safety
Order;” California Health & Safety Code sections 117600 – 118360
(“Medical Waste Management Act); the Resource Conservation and
Recovery Act of 1976 (RCRA); and all other related laws, regulations and
ordinances governing the environmentally responsible management of
bio-hazardous and pharmaceutical waste.
Contractor must be able to furnish Canyon Springs a minimum of three (3)
references who can confirm Contractor has provided services similar to
those required under this contract.
5.
BIOHAZARDOUS WASTE CONTAINERS
Contractor shall furnish Canyon Springs each week two (2) approved
standard 44-gallon containers with tight fitting lids for storage of biohazardous waste at Canyon Springs (between weekly pick-ups). The
containers must be clearly labeled with the bio-hazardous sign or label.
Canyon Springs' personnel will put bio-hazardous materials in 4 mils thick
red plastic bags; those bags will be placed inside the bio-hazardous waste
containers furnished by Contractor. Contractor shall replace picked-up
containers with clean and disinfected containers.
Contractor is responsible for informing Canyon Springs of any changes
regarding required methods for storing medical and/or pharmaceutical
waste.
6.
PHARMACEUTICAL WASTE CONTAINERS
Canyon Springs’ personnel will ensure that pharmaceutical waste is
segregated for storage, and when placed in a container or secondary

container, that container shall be labeled with the words “INCINERATION
ONLY” on the lid and on the sides, so as to be visible from any lateral
direction.
Canyon Springs will furnish the containers for pharmaceutical waste. If
Contractor has specific requirements for containers for pharmaceutical
waste, Contractor will notify Canyon Springs prior to implementation of this
agreement.
7.
SCHEDULING
Contractor will pick up and transport biohazardous waste from Canyon
Springs at least every seven (7) days, or more frequently as requested by
the CS Contract Manager. Pick ups shall be scheduled during normal
business hours, Monday through Friday, excluding designated State
holidays, between the hours of 8:30 a.m. and 4:00 p.m.
Contractor agrees to schedule pick-ups as specified by the CS Contract
Manager. The CS Contract Manager will identify a specific day of the
week for Contractor to provide the required service. If the Contractor has
to alter the regular pick-up schedule, Contractor will ensure biohazardous
waste is not stored for more than seven (7) days between pick-ups, and
the altered schedule does not result in an additional cost/charge to
Canyon Springs.
8.
PICK-UPS
Canyon Springs does not have a loading dock; therefore, any truck used
for pick-ups should be equipped with a tail lift, or other mechanism, to lift
items from/to the ground level.
Upon arrival at Canyon Springs for the weekly pick up,
Contractor/designee shall check in with the Receptionist. Receptionist will
contact a Canyon Springs’ staff member to escort the Contractor/designee
to the biohazardous storage area. Contractor/designee will be responsible
for relocating the waste container(s) from the storage area to the
Contractor’s truck.
Contractor/designee will verify with the Canyon Springs’ receptionist or
Pharmacist, before leaving Canyon Springs, whether or not there is any
pharmaceutical waste to be picked up that day.
Before leaving Canyon Springs, Contractor/designee will leave a receipt or

tracking manifest sheet that documents items that were picked up, or
documents that there was no waste picked up. Documentation shall be
left at the Reception counter, or with the Canyon Springs’ Business
Manager.
9.
RECORD KEEPING
Contractor shall record and provide legible receipt(s) and/or tracking
manifest sheets for picked-up bio-hazardous and pharmaceutical waste by
weight in pounds and number of containers. Documentation shall include
the date of pick up, and identify the treatment facility (name and address)
the Contractor shall transport the waste to. Contractor shall be able to
provide Canyon Springs with duplicate copies of receipts and/or tracking
manifest sheets, as requested by the Canyon Springs’ Contract Manager.
Contractor shall be responsible for retaining documentation related to the
pick-up, storage, treatment, and/or destruction of all waste picked up from
Canyon Springs, as required by statute/regulation(s).
Contractor shall provide Canyon Springs with a supply of any required bar
code labels for biohazardous or pharmaceutical waste containers.
10.
REGULATORY CHANGES
Contractor shall inform Canyon Springs of any changes in regulations
affecting the on-site storage of medical, biohazardous, and/or
pharmaceutical waste that may require a change in Canyon Springs’
procedures.
