We run many clinics for the management of chronic diseases such as asthma and diabetes and offer a wide variety of other medical services including antenatal and postnatal care, minor surgery, childhood vaccinations and well-person check-ups. This service is for emergencies which occur after 6pm and before 8:30am. The out of hours doctor can be contacted on: 02838399201. We must stress that this service is for emergencies which cannot wait until the next day. NHS Direct In the case of urgent need when the practice is closed you can call NHS Direct (www.nhsdirect.nhs.uk/) (0845 4647) to speak to a triage nurse. Your needs will be assessed and advice offered or arrangements made for you to see a doctor. Please do not ask to see a doctor out of hours unless you genuinely cannot wait until the surgery re-opens. Call 999 in an emergency. Chest pains and / or shortness of breath constitute an emergency. Appointments Consultations are by appointment and can be booked from 08:30am each day. These can be made by telephone or in person. Patients who are working can pre-book the first appointment the next day. patients have the right to request an appointment with a preferred doctor Disabled Patients The surgery is on the ground floor allowing easy access for disabled patients. Home Visits If possible please try to telephone reception before 10am if you require a home visit. A doctor or nurse may phone you back as it may be that your problem can be dealt with by telephone advice, or that it would be more appropriate to send a nurse, or indeed arrange a hospital attendance. House visits are only available for patients who are housebound because of illness or disability. Please remember that several patients can be seen in the practice in the time that it takes to make one home visit. There are also better facilities for examining and treating patients at the Health Centre. Repeat Prescriptions Please arrange a repeat prescription before your present medicine runs out. You can do this by telephoning the receptionist. She will then get the doctor to prepare a prescription for you. Repeat prescriptions should be ordered 24 hours in advance of collection. Please allow 48 hours, excluding weekends and Bank Holidays, for your request to be processed. Any problems please telephone the surgery. Clinics & Services Clinics & Services We offer the following services: Antenatal Clinic - Tuesday morning Child Health Surveillance Clinic – Wednesday morning Minor Surgery - by appointment with doctor Well Woman Clinic – by appointment Contraceptive - any surgery services Diabetic Clinic - first Thursday every 3 months Nurse Clinics Our nurses are qualified to carry out: Health promotion Blood pressure checks Ear syringing Taking blood samples Immunistions Dressing and removal of sutures Anti-coagulation monitoring District Nurse:Our district nurses are able to provide a broad range of services to those who are over 75 years and those unable to attend the surgery. Health Visitor:Our health visitor is particularly experienced in health matters relating to under fives. Midwife:Our midwives work with the doctors to provide care for mothers before and after delivery and care for the baby in the first fortnight of life. Test Results Test Results Please call between 11am and 3.30pm to enquire about your test results as our reception staff will have more time to deal with your request between these times. Note that the practice has a strict policy regarding confidentiality and data protection and we will only release test results to the person to whom they relate unless that person has given prior permission for the release of this data or they are not capable of understanding the results. When you take your test you will be told how long it will be before the results are returned to the practice. It is your responsibility to check your results and to make an appointment to discuss them with your doctor if your are advised to do so. Self Care Information Registration New Patient Registration The doctors welcome new patients who live within our practice area. As it often takes sometime for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire and are offered a consultation with the doctor. Medical treatment is available from the date of registration. Please contact reception for further information. Confidentiality Confidentiality The practice complies with Data Protection and Access to Medical Records legislation. Identifiable information about you will be shared with others in the following circumstances: To provide further medical treatment for you e.g. from district nurses and hospital services. To help you get other services e.g. from the social work department. This requires your consent. When we have a duty to others e.g. in child protection cases anonymised patient information will also be used at local and national level to help the Health Board and Government plan services e.g. for diabetic care. If you do not wish anonymous information about you to be used in such a way, please let us know. Reception and administration staff require access to your medical records in order to do their jobs. These members of staff are bound by the same rules of confidentiality as the medical staff. However, we are aware that things can go wrong resulting in a patient feeling that they have a genuine cause for complaint. If this is so, we would wish for the matter to be settled as quickly, and as amicably, as possible. To pursue a complaint please contact the practice manager and she will deal with your concerns appropriately. Further written information is available on the complaints procedure from reception.
