Scholarship Request Form
The mission of the Spring Lake Park Traveling Basketball Program is to provide the youth in the Spring
Lake Park School District #16 attendance area the opportunity to learn and enjoy the game of basketball
at a competitive level. The purpose of SLPTBC scholarships is to make it economically possible for
players to participate in the traveling basketball program. Scholarships for up to ½ the registration fee
may be available for players. To be eligible, players must qualify for free or reduced price meals. If
requested, student must be able to supply proof of free/reduced meals (e.g. letter from school district).
The remaining fee must be paid at registration unless other arrangements are made with club.
Where the players have been awarded scholarships by SLPTBC it is expected that the parent(s)
contribute up to four additional hours beyond the standard 8-hour volunteer requirement working at
the SLPTBC tournaments in November and December to help offset the registration costs that are borne
by the SLPTBC organization.
Submit the completed scholarship form prior to completing the online registration. If approved, you
will be provided with a discount code to use when completing the online registration process. Please
note that it usually takes 3-5 days upon receipt of the scholarship request to process it.
