Thesises of speech of Minister of healthcare and social development in response to report of akim of Almaty oblst on 18.02.2015 Dear Amandyk Gabbassovich and the meeting attendees! As you know on February 11 of the current year the extended session of the Government was held under the chairmanship of Head of state at which he gave his appraisal of the current situation and set to the Government, local executive bodies and National companies a number of missions concerning the provision of the economy stability in social sphere. Measures aimed at provision of population employement becoming especially topical in todays conditions ran like a golden tread through the speech of Head od state. As Nursultan Abishevitch noted now it is very important for us not to allow the growth of unemployment and decline in real income of population. In this connection, to coordinate the measures aimed at provision of the population employment and prevention of the unemployment growth the Government has worked out the following protocol of actions. First. Prevention of stoppage in production and the workers' layoff. For this area the map of social strain risks was generated jointly with akims of the regions and monitoring of township-forming and big eneterprises is executed on permanent basis. This make it possible to response operatively to problematic issues and take measures to solve them. As at the beginning of year the monitoring was conducted in 114 enterprises with up to 161 thousand of the employees. Simulataneously the measures were taken to prevent the layoff of workers. This consists in provision of charging the local enterprises with the orders that will assure the обеспечение загрузки местных предприятий заказами, что обеспечит preservationn of jobs. In the beginning of February of this year the Government members visited all regions of the country where they had meeting with management of the big companies, trade unions and akims of oblasts, Almaty and Astana. Within the framework of these trips 102 memorandums were signed between employers, trade unions and akimats to make itt possible to preserve 259 thousand of work places. Totally as of today about 19 thousand od such memorandums are signed with coverage of more than 1,2 million of people that constitutes about 14% of all employed population of the country. At that it was managed to cover more than 40% of all big enterprises (with number of more than 250 persons). In Almaty oblast more tham 140 memorandums were concluded that confirms the acceptance of intents to prevent the layoff of almost 12,5 thousand workers of the oblast. This work is required to be activated. Those initiatives on cooperation which we stipulate within the framework of memorandums will make it possible to level in appropriate extend the todays economic challenges. I am sure that joint efforts of the state, business and representatives of the workers on mutually beneficial conditions will give, on the one hand, the possibility to prevent the decrease of population employment and, on the other hand, will bring sufficient support to business in hard times. This work will be continued with permanent monitoring of fulfilling the liabilities undertaken by the parties. Second, active measures for promotion of employment are expected to be taken in relation to both the unemployed persons and those self-employed in unproductive labor the number of which accounts for 1,2 million of persons. 1 Today these factors are determinant on the labor market and require the system work at the local level. The huge potential is surely contained in the state program "Nurly Zhol" within framework of which the employment of about 400 thousand of people is planned. Taking into account other sectoral and state programs such as housing construction, industrialization program, reconstruction of housing and utility sector and others we have fine opportunity to provide the employment of Kazakhstan citizens and prevent the growth of unemployment. In this area following the instruction of Head of state the complex plans of the population employment will be taken in all regions with taking into consideration the Road map of employment 2020 in conditions of special perios economy. Since 2011 for the period of its implemetation it makes it possible to involve into active measures of the employment promotion 461 thousan of persons belonging to the category of lowerincome, unemployed ans self-employed persons. This had a positive effect on the unemployment level which according to the results of 4 quarter of 2014 constituted 5,0 %, and decreased by 0,8 percentage points as compared with 2010. Concerning the first area within the framework of Employment road map 2020 55 thousand work places were created in 4 thousand infrastructure projects in rural area. Concerning the second area. From the beginning of Employment road map 2020 implementation 28,7 thousand of people have obtained microcredits among which 20 thousand firstly started their own business. Additionally 27 thousand work places were created in rural area. Concerning the third area 121 thousand of persons are covered by vocational training. 105 thousand persons from among thoses who completed their training 87% are employed. 132,5 thousand of persons were sent to state-subsidized work places (SRM and PM) 67% of which received permanent jobs. Within the framework of increasing mobility of the labor resources, construction of 1 295 houses (4 884 appartments) have been completed since the beginning of implementation. Участниками Программы и членами их семей заселено 1 244 houses (4 729 appartments) or 96,1% of commissioned ones were accomodated by the Program participants and their families' members. Construction of 42 houses (493 appartments) is in the process. In total for these years about 16 thousand of people were rehoused. 78,3% from the composition of the rehoused able-bodied were employed in their new communities. The analysis shows that the measures conducted within the Program framework had positive effect on the situation at the labor market in the republic. Share of the self-employed in composition of the employed population decresed from 30,3% (in 2013) down to 29,4% (4 quarter of 2014). For the specified period number of unproductively employed decreased from 922 thousand persons down to 720 thousand. Implementation of the youth practice and creation of social work places program contribute in prevention of the youth unemployment growth. As of reporting date its index is formed on the level of 4% (in age of 15-28 years). Similar situation is also characteristic for indices of the labor market state in oblast. In whole the statistics according to the resulta of 4 quarter of. 2014 confirms the positive dynamics of almost all indices of the labor market. However, it is necessary to pay attention to the following tendencies: - within recent years the level of unemployment consistently decreases; at that the unemployment in rural area increases in comparison with 4 quarter of 2013 (by 0,1 of p.p .) and 2 continues to be exceeding the average republican level for 0,4 p.p. (4 quarter of.2014, oblast – 5,1%, the RK – 4,7%); Employment road map 2020 is very successfully implemented in oblas; results of its implementation have been already noted in speech of Amandyk Gabbassovitch. Let me continue and pass on to the third action of the Government algotythm optimization of the involved man power structure will be conducted. Necessity of undertaking such measures is dictated by condition of the domestic labor market, as well as by current foreign policy and economic situation in the neighboring Russia and its recent legislative initiatives in the sphere of migration. As of today the quota for involving the foreign man power (hereinafter – FMP) in the Republic of Kazakhstan is established at the rate of 0,7% to economically active population (63 thousand pax), at that 30,7 thousand work permissions issued to employers (legal persons) are applicable. Main country of qualified FMP origin from among thefar-abroad countries id China (9,6 thousand of people or 32% of all applicable permissions), and among the CIS countries – Uzbekistan (1,1 thousand of people). As for the natural persons, they involve 72,6 thousand of persons from CIS for the work in private househols including citizens of Uzbekistan – 69,2 thousand (95%). From among other countries of Central Asia region the greatest in terest in using the tools of labor migration is expressed by the citizens of Tadjikistan and Kyrgyzstan. Under conditions when there is retention of risk related to increase of labor migrant coming and with the purpose of preventing the disbalance on the internal labor market the working group is created to develop the set of measures aimed at regulation of the migration processes. As of today the following proposals are worked out. They stipulate - strengthening of migration control. This means the raid actions conducting by the authorities of migration police, first of all, at the construction facilities, entities of agroindustrial complex and trade; -control over conformity of the arrival purpose with actual activities in relation to every foreigner by means of information-and-analytical systems; - improvement of conditions and procedures of involving the foreign man power via tools of quoting, issue of work permissions and requirements for preparation of the domestic personnel; - prohibition for the labor migrants at the projects implemented at the expense of state budget. Taking into consideration the draft law on migration being in the Mazhilis of Parliament these actions will make it possible to improve procedures of FMP involvement as well as adapt the domestic labor market to current economic conditions and challenges. Forth Retention of social obligations in relation to population In the course of extended meeting of the Governement Head of state determines the particular measures of economic policy for current year and mid-term perspective. President of Kazakhstan called the optimization of budget expenses as one of key elements of crisis response measures but at that he noted "social obligations towards the nation shall not be affected". This year for financing the programs of social development and sphere of healthcare the expenses at the amount of KZT 2,4 trillion is provided for in the republican budget. This sum includes: - for healthcare KZT 678 billions will be directed (for provision and extension of the guaranteed volume of free medical care - KZT 574 billion); - for social payments – KZT 1,6 trillion. Government will execute in full volume the undertaken social obligations. Starting from January 1, 2015 all amounts of pension payments are increased by 9%, amounts of base pension and benefits are increased by 7%, - this will cover 4 million of receivers. Timeliness of social payments at the local level is provided. Fifth, sphere of healthcare 3 This year the implementation of "Salamatty Kazakhstan" State program is expected to be completed. In direct care new medical technologies are actively used. National screening program allowing detection of the diseases at early stages is implemented. On the stationary stage the Unified National system of healthcare has already acted; since the last year its implementation on the outpatient and polyclinic level is started. As a result of conducted measures, - differentiation of conditions and difference in financing the primary medical and sanitary aid between regions are overcomed. Since 2014 the Integrated capitation standard rate of primary health care financing is introduced in the country (today it constitutes KZT 862on the average for the republic), - on the basis of "money after patient" principle the availability of stationary aid to the country citizens was extended, not depending on the place of their living. Only for last two years more than 34 thousand of rural people received highly specialized medical care in republican and oblast hospitals, - modern medical centers implementing new technologies and kinds of treatment according to profile of disease in practice are created. The experience accumulated in the Centers is widely used everywhere in the regions. Currently in the number of oblast and municipal hospitals operations on heart and on the brain are successfully performed, transplantation of vitals is executed. Only for the last year almost 47 thousand high technology operations were performed in the country. It makes it possible to extende the expected life duration of Kazakhstan citizens and lower the indices of mortality including maternal and infant mortality. Today following the instruction of Head of state we started to develop the second stage of "Salamatty Kazakhstan" State program of healthcare development for 2016-2020; its main areas will be: First. Provision of financial sustainability of the healthcare system. From 2017 in Kaakhstan based on the Unified National Standard of Healthcare the social medical insurance will be implemented stage-by-stage, that will allow to establish the multi-channel principle of the healthcare financing at the expense of state, employer, citizen and other sources and provide joint and several liability of the system participants for the health security. Second. Development of primary medical and sanitary aid (hereinafter – PMSA) aimed at prevention and early detection of the diseases. Today in Almaty the first geagraphically remote office of WHO for PMSA is opened that entrust great obligations in development of PMSA not only in our country but in all european region, that is for 53 member states of WHO. Third. Development of the workforce capacity in healthcare. Also under consideration is the issue of implementing the international standards of medical education and scientific research. Medical universities of the country shall become "Centers of competence", the researches should be conducted by scientific consortiums. Thank you for attention 4
