Children entering Kindergarten must be five (5) years of age on or before January 1, 2016. All children who were born in 2009 are eligible for Kindergarten in the Fall of 2015. Two (2) proofs of Danbury residency: One MUST BE a current property tax bill, mortgage deed, or a notarized current rental lease. A current gas/electric/cable bill can be used as the second proof. 2. The child's original birth certificate. 3. Proof of completed immunizations: The CT State Department of Education Health Form must be filled out by your child’s health professional and be on blue paper. At time of registration, this form can be supplied, if needed. The last dose must be given on or after the 4th birthday Polio 3 At least 3 doses. The last dose must be given on or after the 4th birthday MMR 2 2 doses separated by at least 28 days, one dose on or after the 1st birthday. Hepatitis B 3 3 doses, the last dose on or after 24 weeks of age Varicella 2 2 doses separated by at least 3 months. 1st dose on or after the first birthday or verification of disease. Hib 1 One dose on or after the 1st birthday for children less than 5 years-old Pneumococcal 1 One dose on or after the first birthday for children less than 5 years-old Hepatitis A 2 Two doses given six calendar months apart, 1 st dose on or after the first birthday. Verification of the diseases: Immunizations In Progress Laboratory confirmation of immunity is only acceptable for Hepatitis B, Hepatitis A, Measles, Mumps, Rubella, and Varicella. Verification of Varicella: confirmation in writing by a MD, PA, or APRN that the child has a previous history of disease, based on family or medical history. In those instances at school entry where a school-aged child is not adequately immunized school attendance shall be permitted only if that child: (1) has received a dose of each required vaccine for which that child is behind in the month prior to first attendance; and (2) continues on the catch-up schedule recommended in the most recent standards of care for immunizations in Connecticut. A proof of completed immunizations, with specific dates for Polio, Measles, Mumps, Rubella, Varicella,(or verification of disease), HIB for children less than 5 years old, Hepatitis B, Hepatitis A, and DTaP is part of the required registration packet. Please bring any other pertinent health records. The CT State Department of Education requires proof of immunization and a completed physical, completed within 12 months of the start of school, be turned into the school nurse BEFORE the child begins school. Transfer Students: If a student transfers into a Connecticut school from out-of state, no matter the grade, proof of immunizations and a physical must be presented before entrance into school. The blue medical form must be completed and can be obtained by calling the school your child will be attending. Home Schooling: Any parent who wishes to Home-School their child must notify Dr. William Glass at the District Office at 63 Beaver Brook Road at 797-4724, starting at age five. CT General Statute 10-184 has changed the mandatory school age from seven to five. However, parents can sign a waiver at age five and then again at age six at their home school.
