Please bring an original state certified birth certificate with the state seal, as per requirement of WV State Law. Information for acquiring West Virginia documentation can be obtained at your county court house. Also, please bring proof of residency and a social security card. EVERY CHILD ENROLLING MUST HAVE A COMPREHENSIVE HEALTH SCREENING as well as proof of a DENTAL EXAMINATION COMPLETED Health Check Forms are available at local physicians’ offices, the Jefferson County Board of Education office and the Pupil Services webpage, under Medical Forms. There is no specific form for the dental exam – at a minimum, proof of exam should show the date of the exam (must be within the previous 12 months) and have the dentist’s signature. REQUIRED IMMUNIZATIONS Please be prepared to show proper documentation. -DPT – Four doses required. One dose must be after the 4th birthday. -Polio – Three doses required. One dose must be after the 4th birthday. -Measles, Mumps & Rubella (MMR) – Two doses required. First dose must be after the 1st birthday. -Varicella (Chickenpox) – Two doses required. First dose must be after the 1st birthday. -Hepatitis B – Three doses required. Last dose must be after the age of 6 months. -Tuberculin Test - Only required for students in high risk categories. See Jefferson County Health Department for screening information. * To find out how to get immunization records, contact your family physician or contact the Jefferson County Health Department at 304-728-8416. The Health Department holds monthly immunization clinics. A child must be 5 years old before September 1st to enter Kindergarten School policy allows admission of children born between September 1st and September 15th inclusive only if they meet eligibility requirements. To be eligible for early Kindergarten entrance, the child must score at or above the ninety fifth percentile on a standardized comprehensive measure of developmental skills when compared to age norms appropriate to a child who would be 5 years old prior to September 1. This assessment will be administered by qualified Jefferson County personnel. To enroll a child born between those dates, please contact the Pupil Services Department at 304-728-9236.
