The following is needed for ALL incoming students regardless of grade level
BIRTH CERTIFICATE - You must provide your child’s birth certificate if he/she is enrolling in
Kindergarten or is new to the United States.
IMMUNIZATION RECORDS - All immunizations must be up-to-date. Please see the back of this
form for a list of necessary immunizations. If this is the first time your child is enrolling in a
California school he/she will need a recent Mantoux TB screening (within 1 year of 1st day of
school.)
PROOF OF RESIDENCY - Your valid California driver’s license or ID card with current address
and one current utility bill. (gas, electric, water, or trash bills only.) TELEPHONE BILLS ARE
NOT ACCEPTED. If you reside with another person whose name appears on the household’s
utility bills, please ask for a Responsibility of Utility form.
Enrollment for new students in 1st grade must include all of the above AND:
DENTAL SCREENING – Your child must have dental exam within 12 months of entering school
in either Kindergarten or First Grade.
PHYSICAL – Your child must have a physical form, completed by your doctor, within 18 months
of entering First Grade.
TRANSFER REPORT – Please provide proof of transfer from your child’s last school.
Enrollment for new students in grades 2-5 must include the following:
 Birth Certificate
 Proof of Residency
 Immunization Records
 Transfer Report
SCHOOL HOURS FOR ALL GRADES ARE: Mon, Tues, Thurs & Fri 8:15am2:32pm Every Wed 8:15am-1:00pm
Students enrolling in Transitional Kindergarten for the 2016-2017 school year,
must have been born between September 2, 2011 through December 2, 2011.
Students enrolling in Kindergarten for the 2016-2017 school year, must have
been born on or before September 1, 2011.
California State Law prohibits the enrollment of students who have not complied with the
necessary immunizations laws. The law requires immunizations against the following vaccines:
Polio, Diphtheria, Tetanus and Pertussis (DTP/DTap/DT/Td) (Tdap= Tetanus toxoid, reduced
diphtheria toxoid and acellular pertussis vaccine), Measles, Mumps, Rubella (MMR), Hepatitis
B and Varicella. The chart below provides the required doses for each vaccine:
IMMUNIZATIONS
Polio - 4 doses; 3 if last dose was given after 4th birthday
DTP/DTap/DT/Td - 5 doses anytime; 4 if last dose was given after 4th birthday
Tdap – Required for 7th grade entry
MMR – 2 doses after 1st birthday
Hep B or HBV- 3 doses (3rd dose must be separated from 1st dose by at least 16
weeks)
Varicella – 1 dose; 2 doses if new to CA and if child is older than 13
In addition to the above immuinzations, all children entering 1st grade are required to have a
Health Examination within 18 months prior to entry. Please give the attatched form to your
health care provider to complete.
California law, Education Code Section 49452.8, now requires that your child have an oral
health assessment (dental check-up) by May 31 in either kindergarten or 1st grade, whichever
is his or her first year in public school. Assessments that have happened within the 12 months
before your child enters school also meet this requirement. The law specifies that the
assessment must be done by a licensed dentist or other licensed or registered dental health
professional.
