The Wisconsin Medical Home Initiative promotes children’s health and development
through its direct support to primary care clinicians and families. Training, technical assistance
and resources are provided to support medical home implementation with a focus on the topics
below.
Developmental Screening
Clinicians participate in sessions delivered onsite at clinics to promote the early identification of
and timely referral for children with delays. Community partners serving children with delays
share information about their programs.

Behavioral Health Integration
Clinicians select behavioral health-related topics on which they wish to receive customized
support including pediatric mental health screening, community resources, and maximizing
reimbursement for behavioral health provision in primary care settings.

Family Partnership
Learning opportunities are offered for physicians, their care teams, and families on medical
home topics. Discussions are focused on how families and their doctors can work together to
make comprehensive care in the community a reality.
