Both pages of this form and all accompanying documentation must be sent by Chapter Historians to the Division
Historian by the Division deadline. The Division Chapter Where No Division Historian must send the required Division
reports to the Historian General by: September 1. The statistics will be used by the Historian General to determine the
winners of the General historical awards listed in "Chapter 9 - Annual Convention" in the UDC Handbook.

observed by students
2. Essays written by students for competition in UDC essay contests
3. Jefferson Davis Historical Silver Medal presented to students.

Chapter Yearbooks compiled which include a listing of Historian General's programs.
Only the winning Chapter Yearbook from the Division should be taken to the
General Convention for judging. NO Chapter Yearbooks are to be mailed to the
Historian General.
Chapter or Chapter Where No Division Scrapbook compiled. Chapter scrapbooks
must have been judged at Division Convention. Only the winning Chapter Scrapbook
shall be brought to General Convention for judging
Confederate era artifacts, relics, written material collected and presented to UDC
General, Division, or Chapter archives or museums. Attach list of items and names,
location of museums.
Number of CDs, DVDs or computer software on Confederate or
Southern history donated to schools, libraries, museums, or State
Archives. Attach list of items, name of museum or school to which
donated.
Report dollar value of each item.
20. Books on Confederate or Southern history or southern biographies
