Qualifications: Bachelor’s degree in social work or related field. Minimum two years
experience in serving homeless families and/or individuals in crisis. Excellent
interpersonal skills and ability to interact effectively with other agencies, and a familiarity
with issues of substance abuse and domestic violence. Bilingual English/Spanish
required. Understanding of the issues faced by low-income populations. Must be able to
work in a face pace environment. Reliable transportation and valid driver’s license
required.
Responsibilities: The Case Manager assists Bridges clients and former residents of
Shelter Network’s singles and family programs to maintain stable housing through homebased programs. The worker also provides case management to some in-house clients,
helps with special events, and assists with administrative duties as needed.
Duties:
1. Case Management: Provide case management services to families and individuals
who have entered our long term Transitional Bridges Program (scattered sites) and
those who have graduated from the transitional housing programs. Conduct home
visits. Intakes, assessments, and weekly appointments to assist participants meet their
goals. Provide information and referral as needed; crisis intervention. Assess needs,
establish service contract specifying goals and tasks, provide information and referral,
job development, housing search, life skills training, crisis intervention, and advocacy
as needed. Keep case files up-to-date, thorough, and accurate. Provide case
management services to 2-3 in-house clients with the goal of transitioning to
permanent housing.
2. Program Development: Assist Program Director in planning and implementing
house meetings, support groups, and special events. Participate in program
development, both within the program and agency-wide. Develop and maintain
strong community ties and help clients productively engage the community and
maximize available resources. Maintain Bridges and Follow-up statistics and assist in
preparing reports.
3. Job Development: Assess resident’s work readiness, make referrals, and provide job
search skills training. Create and maintain job listings. Develop contacts with
potential employers. Maintain professional relationships with referral agencies.
4. Meetings: Meet weekly with staff to coordinate delivery of services. Meet weekly
with Program Director for supervision. Participate in meetings with other community
agencies. Attend All Staff meetings and other meetings as requested. Fill other
reasonable requests as made by supervisor, including assistance across program lines.
5. Other: Assist in maintaining a clean and safe environment. Other duties as assigned.
