3 day Mapping Patient Pathways and Care Pathway Document Design
Course summary:
This new three-day course has been designed for those people wishing to gain the knowledge and skills to both
map patient pathways using live electronic capture, and to design decision scorecards and care pathway
documents that prompt patient-centred, locally agreed, evidence-based best practice at the front line and that
compare planned with actual activity, outcome and performance.
The first two days of the course cover all aspects of the development of the care pathway journey/ patient pathway
including preparation for, and design of, activity-based process mapping workshops and specific facilitation and
capture techniques. Practical workshop exercises develop the skills required to organize and facilitate effective
care pathway activity-based process mapping workshops, delivering tangible and measurable service improvement
and re-design. Training and workshops cover the skills required to use live electronic mapping to capture and share
process, outcome and performance. The course includes a complimentary single-user version of the Nimbus
Pathway process mapping and performance management software.
The third day of the course provides practical workshops to design local care pathway templates and content. The
day covers all aspects of the writing and laying out of content for care pathway documents. Practical workshops
develop the skills required to construct meaningful and effective care pathway content that will demonstrate the
consistent delivery of locally agreed evidence-based best practice at the frontline. By applying strict rules and
principles for the writing of care pathway document content coupled with well thought out layout within the unique
care pathway variance tracking mechanism, delegates will learn to develop high quality, effective care pathway
documents. Delegates will gather the skills to use the information captured in these documents to feed the many
national and local agendas and initiatives such as integrated governance, risk management, clinical coding and
other corporate performance reporting initiatives, as a bi-product of routine record keeping. This approach enables
organisations to move from fragmented corporate performance reporting conducted in silos, to integrated,
consistent, accurate and timely corporate performance management.
We strongly advise attending the one-day Care Pathway Foundation Course prior to this course. Courses can also
be provided in-house and tailored to meet your local needs.
