POST: SPECIALTY DOCTOR IN NEONATAL MEDICINE AND NEONATAL TRANSPORT

Applications are invited for this newly created Specialty Doctor post within the Glasgow
neonatal services. The post provides the opportunity to establish a career within fully
comprehensive neonatal service where all the neonatal clinical services can be
accessed on site. The post will initially work between the Southern General and
Yorkhill Hospital neonatal units and the West of Scotland neonatal transport service.
The Yorkhill neonatal unit is a combined medical & surgical neonatal unit within the
regional children’s hospital. A full range of neonatal services and support services are
available on site including neonatal surgery, cardiac surgery, renal support and the
Scottish neonatal ECMO service. Over 200 newborns per year are currently
transferred here due to the specialist services offered by this site.
The Southern General neonatal unit currently operates with 6 ITU, 6 HDU cots and 12
special care cots. We are in the process of a major redesign of maternity, neonatal and
paediatric services in Glasgow. This started at the beginning of 2010 with the closure
of the Queen Mother’s Maternity Unit, transfer of its services to the Southern General
site and the opening of new-built labour ward and new neonatal unit. In 2014/15 the
Children’s Hospital will be relocated to new built facilities on the SGH site at which time
the new neonatal unit here will become the largest neonatal unit in the country with 34
intensive care and high dependency cots and 26 special care cots providing the full
range of neonatal services including neonatal and cardiac surgery and ECMO.
The Neonatal Transport Service (West of Scotland) is based in Glasgow and currently
undertakes 800 transfers per annum by road and air, including the transfer of critically
ill infants for ECMO and neonatal surgery. The West of Scotland Neonatal Transport
service has the most experience of air transfers in the UK and has a CAA approved
system for delivering iNO in the air.
The Specialty Doctor will be expected to work alongside senior neonatal trainees
within the department providing support to more junior middle-grade and first on-call
colleagues. Individuals will be expected to develop a particular interest or role within
the departments. Applicants will be expected to demonstrate competence in practical
neonatal procedures and neonatal resuscitation, to be familiar with a range of neonatal
ventilatory strategies and to demonstrate clinical leadership abilities.
We are looking for applicants who can demonstrate the capability to work in
Neonatology at middle-grade level and who are enthused by the prospect of career
within our neonatal service. Neonatal transport experience is desirable but is not
essential as training will be provided. There are currently two specialty doctors in post
and the appointment of this third post is intended to allow all three specialty doctors to
develop some involvement within the neonatal transport service probably on a rotating
block attachment of several months at a time.

Neonatology services in Glasgow and Clyde operate as a single service. The posts are
based at the Southern General Hospital, Yorkhill Hospital and the Neonatal Transport
Service and complement the work of the Consultants and middle grade staff. The post is
full time 10 Programmed Activities (PAs) with two Additional Programmed Activities
(APAs) available.
QUALIFICATIONS
MRCPCH or equivalent would be an advantage. The successful candidate should have
considerable experience of neonatal intensive care and neonatal follow-up clinics.
Neonatal Transport experience would be desirable.
The successful candidate will be encouraged to develop a special interest appropriate for
the needs of the department. (Training will be available)

2 current Specialty Doctors working in neonatal unit. The appointment of the third post will
allow three to develop some involvement in the neonatal transport service.
11 middle grade doctors (Specialty trainees or SpR’s on the West of Scotland training
rotation) based between SGH and Yorkhill neonatal units
7 Foundation year 2 trainees and 6 ST2 Paediatric trainees.
Expanding numbers of ANNPs working in both transport and in-patients neonatal
services
Neonatal and Children’s Services across NHS Greater Glasgow and Clyde
Neonatal services across Scotland are currently undergoing a process of change and it is
anticipated that, due in part to workforce pressures, some centralisation of neonatal
intensive care cots in Scotland will occur but the final model of this service has not yet
been decided. In addition there is service redesign locally in Glasgow involving the move
from four neonatal facilities to two. Currently neonatal intensive care is provided in the
Princess Royal Maternity (PRM), the Southern General Maternity (SGH), and a combined
medical and surgical NICU in The Royal Hospital for Sick Children (RHSC) on the Yorkhill
site. In the future, the final location of neonatal intensive care units will be the PRM and
the SGH site, the latter being co-located with the newly built regional children’s hospital.
Following the building of the New Children’s Hospital on the Southern General site
neonatal surgical and sub-speciality services will be provided in the Neonatal Unit which
will be adjacent and connected to the New Children’s Hospital via a link corridor. The
National Neonatal Transport Service (NeTS, Western Region) is currently moving to a
new base at Yorkhill but in the long term will also be based at the SGH site.
All neonatal specialty doctor posts in Glasgow are appointed through the Women and
Children’s Directorate of NHS Greater Glasgow and Clyde. Therefore, following job
planning, the future working pattern may involve other sites within the Directorate.
DUTIES OF THE POST
1. To work with the middle grade staff in the provision of cover to the Neonatal
Intensive Care Units at the Southern General and Yorkhill. Out-of-hours duties are
shared with your middle grade colleagues.
2. To work as a middle grade doctor in the Neonatal Transport Service. Non-resident
on-call is shared with ANNPs.
3. To assist Consultants in Neonatal Outpatient Clinics. To act with a degree of
independence in the provision of an Outpatient Clinic according to the candidate’s
suitability and training.
4. Support of the junior staff in the day-to-day management of patients in The
Neonatal Unit and Postnatal Wards.
5. Teaching of undergraduates and postgraduate medical students, nursing and
other staff.
6. Management duties appropriate to the post.
7. Audit duties, individually and together with other members of the Unit.
8. The post holder will be expected to meet the requirements of CPD through the
RCPCH system.
ROTA
Out-of-hours duties will be split between a 1:12 resident middle-grade shift rota in one of
the neonatal units and a 1:18 on-call rota in neonatal transport. Total weekend
commitments average one weekend per month split between non-resident transport oncall and resident neonatal unit shifts. It is anticipated that at any one time your duties will
be primarily based in either transport and NICU.
CONDITIONS OF THE POST
All periods of duty in the Southern General and Yorkhill are on-site shifts. Transport
Service duties involve on-call from home requiring the ability to return to the hospital base
within an hour of receiving a call.
ANNEX B
Model Job Plan
Specialty Doctor/Associate Specialist Job Planning
1. A model job plan is attached, which has been agreed as the format used by
employers in Scotland to record details of doctors’ or dentists’ working arrangements
under the new contracts.
2. It should be used to confirm the outcome of both annual and interim Job Plan Reviews
and the details contained in the Job Plan will form part of the contract between
employer and employee. Doctors or dentists participating in job planning activities
should be reminded of the important nature of this document.
3. The document must show the total number of Programmed Activities (PAs) (including
those allocated for on-call work) and the number of Additional Programmed Activities
(APAs) separately.
4. To ensure clarity and consistency, each item of work should be categorised as Direct
Clinical Care (DC), Supporting Professional Activity (SPA), Additional NHS
Responsibilities (AR) or External Duties (ED) and each should be listed separately as
either a PA or an APA.
5. It should be noted that some blocks of time may include more than one category of
work, but must not include both PAs and APAs. These must be noted separately to
inform any APA contract that results from the job planning process.
6. It is recommended that a diary should be completed as an evidence gathering tool to
inform the process. To ensure that a representative period is used, it is recommended
that diary evidence should be submitted for a minimum period of six consecutive
weeks or, where practicable, the period of one cycle of on-call.
7. The reference period used for the diary should be agreed in advance between the
doctor or dentist and their clinical manager.

This timetable is indicative only. Post-holders will be expected to spend periods of time
largely committed to each part of the neonatal service. During NICU-based time daytime
midweek duties will be largely based in the neonatal unit and will include 6 long (12.5
hour) shifts every 6 weeks and 1 weekend of Fri, Sat & Sun resident night shifts in every
6 weeks. Whilst based in neonatal transport there is a commitment to a 1:6 non-resident
24-hour on-call rota. These formal out-of-hours commitments are integral to the basic 10
PA allocation. There is rostered midweek time off to compensate for evening and night
shift working.
TERMS AND CONDITIONS OF SERVICE
The conditions of service are those laid down and amended from time to time by the Hospital and Medical
& Dental Whitley Council.

New Entrants to the NHS will normally commence on the minimum point of the
salary scale, (dependent on qualifications and experience). Salary is paid
monthly by Bank Credit Transfer.

SUPERANNUATION
You have the option to join the NHS Superannuation Scheme, to participate in
the State Earnings Related Pension Scheme or to take out a Personal
Pension. Employee’s contributions to the NHS Scheme are Tiered based on
your earnings and the employers contribution equates to 13.5 % of salary.
Employees in the NHS Scheme are “Contracted-out” of the State Earnings
Related Pension Scheme and pay a lower rate of National Insurance
contributions. Employees who choose to participate in the State Earnings
Related Pension Scheme pay the higher rate of National Insurance
contribution. A Stakeholder Pension is also available. A Personal Pension is a
private arrangement agreed with the pension provider that will be an
organisation such as a Bank, Building Society or Insurance Company.
REMOVAL EXPENSES
Assistance with removal and associated expenses may be given and would be
discussed and agreed prior to appointment.
EXPENSES OF

with appropriate travelling expenses. Re-imbursement shall not normally be
made to employees who withdraw their application or refuse an offer of
appointment.
TOBACCO POLICY
NHS Greater Glasgow and Clyde operate a No Smoking Policy in all premises
and grounds.
DISCLOSURE SCOTLAND
This post is considered to be in the category of “Regulated Work” and
therefore requires a Disclosure Scotland Protection of Vulnerable Groups
Scheme (PVG) Membership which currently costs £59.00. The cost of the
PVG Membership will be initially paid by NHS Greater Glasgow and Clyde and
will required to be repaid through a payroll deduction mandate from the
successful candidate’s first salary.
