Declaration
I understand and agree to the following:
1.
My dog where possible will be taken to the vet detailed above. If this is not possible my
dog will be taken to a vet of your choice. I will pay any associated costs on collection of
my dog. I do / do not wish to be contacted if my dog needs to go to the vets.
2.
My dog will spend time in the kennel area and paddock as well as time in your house.
During all of my dogs stay I am happy for him/her to be socialised with other dogs.
3.
My dog may go to dog shows/on holiday with you. If this happens then my dog will be
treated like your own (eg kennelled in your van to sleep, walked on the beech etc).
4.
I confirm that my dog has spent a 24 hour socialisation period with Krazy K9’s prior to my
first kennel booking.
5.
That Krazy K9’s will not be responsible if any re-occurring or historical injuries flare up
during my dogs stay.
6.
That my dog will always be walked on a lead/flex and will not be allowed to run free.
7.
If I have a bitch that comes into season, I understand that Krazy K9’s will do their best to
ensure she does not come into contact with entire male dogs, but cannot guarantee this
or that she won’t fall pregnant. This may mean that exercise is more limited than would
normally be the case.

Our fees are currently £15.00 per night or £10 for a day (not overnight). We will agree with you
closer to the time the drop off and pick up arrangements for your dogs stay. To secure your
booking we ask for a 25% deposit. If you have to cancel the booking providing we are able to
take another booking for the same dates we will refund your money. Please return your
deposit with this form.
