JOB DESCRIPTION JOB TITLE: Registered Nurse SUPERVISOR: Treatment Director or Designee GENERAL STATEMENT OF ROLE: The Nurse provides physicals and immunizations (as appropriate) for designated residents who are admitted to Seven-12 House, Youth Recovery House, and Rosedale Shelter Programs to ensure health and safety of residents served, as well as maintain applicable State licensure requirements. The Registered Nurse will provide medical assistance for the Telehealth Program by checking height, weight and vitals as needed for clients. REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES: Knowledge of current professional nursing theory and practice. Knowledge of current literature, trends and developments in the field of nursing. Knowledge of current specialized client care to be delivered. Knowledge of learning strategies and principles of counseling. Knowledge of current medications, administration, their actions and use. Displays high standards of ethical conduct. Exhibits honesty and integrity. Refrains from theftrelated, dishonest or unethical behavior. Works and communicates with clients and staff to meet their needs in a polite, courteous, and cooperative manner. Committed to quality service. REQUIRED EDUCATION AND EXPERIENCE: Graduation from an accredited Nursing Program and Graduation from an accredited college or university with a Bachelor’s degree in Nursing and experience equal to four years of full-time clinical nursing. OR Graduation from an accredited college or university with a Master’s degree in Nursing and experience equal to two years of full time clinical nursing. OR an equivalent combination of the required education and experience, substituting one year of full-time experience for thirty semester hours or equivalent of education;. NECESSARY SPECIAL REQUIREMENTS Licensure as Registered Nurse (RN) as issued by the Iowa Board of Nursing. JOB ESSENTIAL FUNCTIONS: While performing the duties of this job, the employee is regularly required to talk and hear. The employee frequently is required to sit and use hands to finger, handle, or feel objects, tools, or controls. Nurse Practitioner 1/8/08/CB -1- The employee is required to stand, walk, and access the residential facilities in Ames. Specific vision abilities required by this job include close vision, distance vision, and the ability to adjust focus. In addition, the employee must have the ability to: Practice at all stages of health care continuum. Assess clients and determine priorities of care. Relate therapeutically with clients and effectively with others. Keep own emotions under control and restrain negative behaviors when provoked or when working under conditions of stress. Lift up to 20 pounds. RESPONSIBILITIES: 1. Complete a physical on designated residents who are admitted to Seven-12 House, Youth Recovery House and Rosedale Shelter Programs within one week of admittance into the program. The physical will include: a. Completion and review of medical history and intake form. b. Check for communicable disease symptomatology; e.g., dermatitis, discharge, etc. c. Pulse check. d. Respiration checks. e. Blood pressure check. f. Ua and Hgb. or Hct. g. Physical assessment. h. Vision screen i. Nutritional assessment. 2. Complete a TB screen and recheck on all residents of Seven-12 House and Youth Recovery House within 7 days of admission into the program. 3. Submit a written report in the electronic records system within (2) days of the physical examination. The report will include: name of nurse administering physical examination, date of examination, statement pertaining to general health of resident, statement as to whether there is any communicable disease symptomatology, whether a referral to doctor was made, and reasons for the referral. 4. 5. 6. Administer vaccines to designated residents as deemed appropriate and medically necessary. Provide oversight of the Iowa Vaccines for Children Program which will include patient eligibility screen, vaccine ordering, vaccine handling, vaccine accountability, and other program requirements to ensure YSS can maintain its enrollment as a provider in the Iowa Vaccines for Children Program. Make appropriate medical referral when necessary. 7. Provide medical services for YSS Telehealth Program by checking vitals, height and weight for clients prior to psychiatric appointment with prescribing professional. 8. Complete documentation for Telehealth clients within (2) days of the services. 9. Complete time sheets for payroll and billing purposes. 10. Provide medication consultation and education to staff completing the Medication Management Course. 11. Other duties as assigned.
