Documentation Integrity Team Mandatory Orientation for New Physicians with Admitting Privileges Date: April 19, 2012 Name: Rebecca “Ali” Williams, RN, BSN, CCDS Position: Manager of Documentation Integrity Phone: 864-541-1609 E-mail: rawilliams@srhs.com Proposal Mandatory Documentation Integrity orientation/education for all new physicians with admitting privileges at the Village Hospital and/or Spartanburg Medical Center prior to being credentialed. The 20-40 minute education/ orientation session would include information on the query process and the impact of accurate, concise medical record documentation on Severity of Illness, Risk of Mortality, Reimbursement and Profiling. (Presently, all Allied Health Professionals are mandated to attend education prior to credentialing.) Schedule Physicians will be given the contact information to schedule a time at their convenience. Flexibility will be offered Monday - Friday. Allocation of tasks 1. 2. 3. Medical Staff will notify new physicians within their packet material. Physician will contact Documentation Integrity Team and schedule education. Documentation Integrity Team will notify Medical Staff of Completion and send Certificate. Benefits to Spartanburg Regional and Physicians Agencies use the data hospitals provide to Medicare on the UB-04 to determine severity of illness, risk of mortality, mortality index and complication rates. Not only does the hospital benefit with the previous mentioned, but the geometric length of stay (GLOS) & reimbursement for utilization of services can be positively impacted with certain key terms that are able to be coded to. Thorough documentation helps the hospital defense in RAC denials, legal issues and communication of care amongst the medical providers. Physicians are also profiled by third parties and the information is available to the public.
