POST INFORMATION
The Department Emergency and Medical Services Directorate
This encompasses the departments of Emergency Medicine, General
Internal Medicine (GIM) and its related specialties. There are 144 general medical beds including a 8 bed coronary care unit and 4 bed high dependency unit. The medical wards are specialty based. There are an additional 100 care of the elderly beds including a stroke unit and geriatric
Orthopaedic rehabilitation which are part of
Lanarkshire’s Medicine for the
Elderly Directorate.
The Emergency Medicine Department is responsible for more than 75 000 emergency medicine attendances (of which 25% are children), 12 000 acute admissions and 5 000 new outpatient referrals per year. Acute medical admissions are admitted to the emergency care ward which admits all medical admissions. Patients are initially assessed and stabilised and usually triaged within 24 hours to the appropriate specialty ward as far as is practically possible.
General/Acute Internal Medicine (AIM)
The Medical Assessment Unit (MAU) admits approximately 12 000 new patients per year. These patients are admitted to our 36 bedded medical assessment unit and our 12 bedded combined coronary care and medical high dependency unit. A small number of patients are admitted directly to the stroke ward and adult critical care unit. All patients are assessed by the on call physician on one of the twice daily post take ward rounds. Those patients who are not discharged from MAU within 24-48 hours are triaged to one of 7 specialty wards for continuing care depending on their clinical needs. The acute receiving rota is 1 in 16 with 2 physicians responsible per weekday and 1 each on Saturday and Sunday. On the weekdays, one consultant is on-call for a 24 hour period from 9am, undertaking post receiving ward round from 2pm onwards until early evening. The following morning the post receiving ward round is done by different physician i.e. the receiving physician, patients being admitted after 5.30pm the previous day will be reviewed by this physician and not the on-call physician for that period. The Care of the Elderly physicians take appropriate referrals from the medical take directly into their service. In conjunction with our consultant colleagues in the Emergency Medicine department, we are currently developing alternative pathways for patients who may not require an acute admission. The Monklands Site Improvement Group has identified and supported the development of Acute Internal Medicine as a key part of its work. We propose a major expansion of acute physician numbers to facilitate a number of proposed service developments.

Nephrology.
Nephrology services cover the whole of Lanarkshire with a reference population of approx. 560,000. Inpatient renal services are based at
Monklands Hospital (ward 1) which has a complement of 17 inpatient beds including x2 HDU Renal beds. There is an ongoing expansion of RRT with approximately 195 outpatient haemodialysis patients, 15-20 Peritoneal patients and 200 post transplant patients. General nephrology clinics take place on all three of the acute Lanarkshire Hospital sites, Monklands,
Wishaw and Hairmyres Hospitals. Low clearance clinics are held at
Monklands Hospital and supported by a dedicated multidisciplinary team including Dietician, pre-dialysis nurse and anaemia co-ordinator with access to vascular access co-ordinator and palliative care nurse. Vascular access is supported by a team of vascular surgeons, interventional radiologists, vascular access surveillance nurse and Associate Specialist. Acute renal transplantation is undertaken within Glasgow and combined kidneypancreas transplantation within Edinburgh. Patients return to Lanarkshire for long term follow up at Monklands Hospital 12 months post transplant.
Transplant assessment, including live donor assessment is facilitated by a transplant specialist nurse with Consultant Nephrologist support.
Nephrology on-call us undertaken by the 5 nephrologists (ie 1 in 5 with prospective cover) with ward rounds within the Renal Unit at week-ends.
The Nephrologists are also involved in the Medical Receiving rota (see above) with a base Medical ward (ward 14) separate from the Renal Unit

DUTIES AND RESPONSIBILITIES
Main Duties The post is essentially taking over the clinical responsibility of our retiring
Consultant Nephrologist.
As such, in addition to the retiring nephrologist, the post holder will be joining an established team of 4 Consultant
Nephrologists and 2 Associate Specialists. The post holder will be responsible for the retiring nephrologists Medical and Renal in-patient duties as well as taking on the nephrologists on-call duties (Medical, 1 in 16;
Renal, 1 in 5) and outpatient clinics in a phased manner. An availability supplement is payable for out of hours work. The post holder will also be expected to participate fully in the clinical and educational supervision of junior medical staff with particular reference to training in medicine and nephrology.
Should the candidate apply for solely General/Acute Internal Medicine, the
Job plan would include taking over Medical inpatients and the Medical oncall of the established nephrologists to allow the Nephrologists to take on an increased nephrology workload.
The exact details of the job plan will be discussed with the candidate. An illustrative job plan is included with this job pack.

Impact of
Change
For the purpose of immigration status, overseas doctors and dentists are those who, regardless of where they obtain their primary medical/dental qualification, are not nationals of the European Economic Area (EEA). A list of EEA countries is attached.
On 7 th March 2006 the Department of Health announced changes to the immigration rules for postgraduate doctors and dentists. The changes came into effect on 3 rd April 2006 and mean that all doctors and dentists who wish to work in the UK from outside the EEA will need to meet the requirements of an employment category such as the work permit provisions. New applications for permit-free training visas will no longer be granted for doctors undertaking training posts
Non-EEA doctors and dentists will still be able to come and train in the
UK and are still eligible to apply, in open competition for training posts provided all the other eligibility criteria is met. However, training posts will now be considered employment for immigration purposes. Work permits will only be issued where there is a specific post for the overseas doctors that cannot be filled by someone from the EEA.
