International Baseball Federation Federación Internacional de Béisbol International pre© FIBA – 2007 Edition Page 1/2 Application Procedure for Therapeutic Use Exemption (TUE) Only TUE forms for players identified as part of the IBAF Registered Testing Pool and/or likely to participate in competitions of IBAF are to be sent to the IBAF office in Lausanne. The form must be completed in a LEGIBLE way, in either English or Spanish, using the IBAF forms. The procedure to be followed for a TUE application is explained hereafter: 1 Abbreviated TUE Application 1.1 Beta-2 agonists Formoterol, Salbutamol, Salmeterol and Terbutaline require an abbreviated TUE when administered by inhalation. The applicants must send the abbreviated TUE application form and a medical file provided by a specialist in pneumology or allergy, indicating the duration of the treatment (see the WADA guideline “Asthma - Medical Information to support the Decision of TUEC” at http://www.wada-ama.org/rtecontent/document/asthma_TUEC.pdf). If supporting test results are missing, the application will be rejected. 1.2 Glucocorticosteroids 1.2.1 All glucocorticosteroids are prohibited when administered orally (swallowed), rectally, intravenously or intramuscularly. Their use requires a standard TUE to be approved (or not) by the IBAF TUE Committee. 1.2.2 Except for the routes of administration mentioned under point 3 below, all other routes of administration require an abbreviated TUE. Example : - subcutaneous injections in articular or ligament areas - aerosols The abbreviated TUE application is granted a priori upon receipt by IBAF of the player’s application, on the condition that the abbreviated TUE application form has been correctly filled in and, among other things, that the doctor who administered the injection has indicated the diagnosis and the posology. Urgent medical cases may, however, be taken into consideration, even those for retrospective approval on condition that probative documents are provided. 1.2.3 Preparations containing corticosteroids for local use in the case of disorders such as: - dermatological (creams, lotions, ointments, gels) - nasal (drops, spray) - aural/otic (drops) - buccal cavity (tablets) - ophthalmologic (eye-lotions, creams) are not prohibited and therefore do not require a TUE Created on 14/04/2008 2. Standard TUE Application LICATION Any application requiring the use of a prohibited substance (if there is no therapeutic alternative) which cannot be authorized with an abbreviated application (point 1) will be studied by at least three doctors who are members of the IBAF TUE Committee (TUEC). - Application for the use of amphetamine in case of ADHD (Attention Deficit Hyperactivity Disorder): A complete medical file must be provided containing the treatments during childhood, the current treatment and psychiatric examination. Incomplete/illegible forms/documentation will be returned to the applicant.
