Currently located in the West end of Glasgow, the Royal Hospital for Sick Children is a national leader in the secondary and tertiary health care of children and families. Plans are underway for the to move to a new children’s hospital, located on the Southern General Hospitals campus which due to open in 2015. Applications are invited from individuals interested in joining the hospital’s well-established Paediatric Respiratory Medicine Team. This new consultant post will be the fourth full time appointment creating a team of 7 consultants within the department (5.4WTE). The successful candidate will have an operational lead on the delivery services for children with neuro-disability and a significant input to Cystic Fibrosis services in addition to general respiratory clinical duties. The Respiratory department is one of the biggest departments in the UK. The post holder will join 6 consultant paediatric respiratory colleagues (3 full time NHS, 2 part time NHS, I full time academic) who are comprehensively supported by an extensive team of nurse specialists, respiratory technicians and physiotherapists. The department has anactiveresearch portfolio and is actively involved in undergraduate and postgraduate education. The department offers a high level of tertiary support to a population of 3.1 million. A diagnostic and therapeutic service is offered and the focus of the department in the next few years is to develop a modern managed clinic network for complex paediatric respiratory medicine within the West of Scotland. Application is therefore sought from dynamic individuals with specialist paediatric experience who are ready to join our well-established team. Applicants must have full GMC registration, a licence to practice and be eligible for inclusion in the GMC Specialist Register. Non-UK applicants must demonstrate equivalent experience/training. 2 Section 1 Children’s Services across NHS Greater Glasgow and Clyde Children’s Services are managed within the Women & Children’s Directorate located within the Yorkhill Campus that hosts the Royal Hospital for Sick Children. Neonatal services within the city have recently undergone significant re-organisation and are provided primarily on two sites, namely the Southern General Hospital and Glasgow Royal Maternity Hospital. Babies with surgical or cardiac problems are managed within RHSC. The Royal Hospital for Sick Children, Glasgow The Royal Hospital for Sick Children, Glasgow is the largest paediatric teaching hospital in Scotland. It provides care, not only for children resident within Greater Glasgow & Clyde, but is also a tertiary referral centre for children from the West of Scotland, and in some subspecialties, from the whole of Scotland. There are 260 inpatient and day care / assessment beds within the hospital. There is an integrated intensive / high dependency critical care unit (capacity up to 26 beds) and a retrieval service for critically ill children. On the same floor, adjacent to critical care services is a large fully functional theatre suite. A large number of paediatric medical and surgical subspecialties are represented, including general medical paediatrics, respiratory, endocrinology, nephrology, gastroenterology and nutrition, haemato-oncology, cardiology, neonatology, immunology and infectious disease, dermatology, rheumatology, metabolic medicine, audiology, ophthalmology, ENT surgery, orthopaedic, plastic surgery, maxillo-facial and dental surgery and general paediatric and neonatal surgery. The hospital provides a national Extra Corporeal Life Support service and is the centre for all paediatric cardiac surgery in Scotland. The Radiology Department located within RHSC provides ultrasound, CT, MRI and isotopic studies on site. Diagnostic laboratory facilities are on site include Haematology, Blood Banking, Biochemistry, Microbiology, Virology, Histopathology and Genetics. Section 2 provides detail of NHS Greater Glasgow and Clyde’s planning framework to develop a new children’s hospital (NCH), co-located with a new adult hospital in the South side of Glasgow. Paediatric neurosurgery is located in the Institute of Neurosciences at the Southern General Hospital. Paediatric radiotherapy (under general anaesthetic) is completed at the Beatson Oncology Centre, located at the Gartnavel campus. 3 There is an extensive range of specialist community based children’s services across NHS Greater Glasgow and Clyde. Managed within community health and social care partnerships, these services are integrated with primary care and social care services. UNIVERSITY OF GLASGOW LINKS The Yorkhill academic campus is part of the Division of developmental medicine and is led by Professor Lawrence Weaver. The Department of Child Health was founded in 1924 and is located in the heart of the Royal Hospital for Sick Children. It is responsible for the teaching of paediatrics and child health to undergraduate students, and has major research programmes in Paediatric Respiratory Disease, Paediatric Endocrinology, Paediatric Gastroenterology and Nutrition, Epidemiology and Community Child. In addition to the academic, research and administrative staff, Honorary Clinical Senior Lecturers and Honorary Clinical Lecturers assist in undergraduate teaching and examining, and postgraduate training. Planning for this new build is at an advanced stage. 4 Section 3 The Work of the Respiratory Department The consultant will join a team that provides comprehensive paediatric respiratory secondary and tertiary services to the population of Glasgow and the West of Scotland. In addition, the department supports national paediatric services at Yorkhill. This present post is a new post and is to continue the development of the clinical service. V/Q scanning 5 In-patient facilities are currently spread throughout the hospital. The average number of tertiary in-patients including CF is fifteen to twenty per day. Support is available from a comprehensive range of tertiary level sub-specialists within the hospital, and, from the Department of Diagnostic Imaging with on-site X-ray, ultrasound, CT, MRI, interventional and nuclear medical facilities; the Department of Pathology and the Department of Laboratory Medicine. The service provides training for Specialist Registrars undertaking a general paediatric or subspecialty training. The department has CSAC approval for the full package of RCPCH grid training. Yorkhill is home to several departments of the University of Glasgow that undertake collaborative research. There have been progressive developments in the Respiratory Department in the past few years with the development of dedicated facilities for sleep monitoring and long-term ventilation services and considerable multidisciplinary expansion of the CF service. 2006 saw the installation of new lung function equipment. In early 2011 the sleep service moved to dedicated facilities and incorporated a 4 bed system for full polysomnography. The service interacts with many other secondary, tertiary and quaternary services within Yorkhill. These include the national services in Tracheal surgery, Craniofacial surgery, Cardiology/Cardiac Surgery, ECMO, Renal and Bone Marrow transplantation, tertiary services in ENT, Intensive Care, Neurology and Foetal Medicine and the secondary care paediatric services for Glasgow and Lanarkshire. The presence of foetal medicine service results in referral to Glasgow of the majority of infants with congenital lung malformations. Together with the national airway surgery service this ensures a professionally challenging and satisfying clinical caseload of specialist work. There are 5-10 requests for inpatient consultation a week. Neonatal screening has been in place in Scotland for nearly eight years. An orderly and structure handover of patients occurs with an Adolescent clinic in conjunction with the West of Scotland Adult CF unit. There are approx. 80 patients on ventilator support. Duties of the Post The main priorities of the post will be to provide and enhance consultant input to the regional Managed Clinical Networks in Paediatric Respiratory Medicine and Cystic Fibrosis service through involvement in sub-specialist services. A degree of specialisation has developed within the department.It is anticipated that the successful applicant will become the lead consultant for the development of a service for respiratory problems in children with complex developmental problems. It would be desirable that the post-holder bring particular special interests and/or skills to the unit that would complement the department's current profile. The use of such skills to further develop the department’s activities would be strongly encouragedand it is expected that the successful applicant would contribute to one or two other specialist areas. Interest and experience in complex asthma, non-CF bronchiectasis and CF would be particularly welcome. (i) New patient and follow-up tertiary respiratory referral clinic. All the respiratory consultants have a weekly half-day clinic for new tertiary referrals or triaged referrals from GP’s of children with respiratory problems including recurrent respiratory infection and asthma. Follow-up of patients from the clinic and ward referrals also takes place at this clinic. (ii) Inpatient respiratory duty week With this appointment the consultants will move to a 1 in 5 rota of “on service” weeks with Drs Gibson, Devenny, Davies and this new appointee each doing one of the five weeks and the remaining week being covered by Dr Paton and Dr Cochran on an alternating basis. During the service week the consultant is responsible, along with junior staff, for the daily review of all inpatients and providing an acute consultation service and emergency bronchoscopy cover. There is an established out of hours rota for Respiratory medicine with weekend ward rounds at the end of the service week. 8 (iii) Bronchoscopy and investigation reporting Many of the bronchoscopy procedures will arise from support of PICU and other specialist services within the hospital e.g. Haematology/oncology. The consultant of the week reports lung function and ph studies for that week. Abbreviated and full sleep studies and MSLT are reported by Dr Gibson and Dr Davies. (iv) Consultation service Consultation service for patients with respiratory problems admitted to Yorkhill under the subspecialty services and by telephone for similar patients cared for by consultant paediatricians elsewhere in the West of Scotland. This often occurs during the “on service” week, although referrals of a less urgent nature are also frequent. There will be development over the next five years of the Managed Clinical Network (MCN) for complex respiratory disease. The new and existing staff will have a major role in this development. This will include outreach clinics in the District General Hospitals involved in the network (Forth Valley, Wishaw Hospital, Lanarkshire, Royal Alexandra Hospital Paisley, Crosshouse Hospital, Ayrshire, and Dumfries and Galloway Royal Infirmary, Dumfries) (v) Research, audit and measurement of clinical performance Participation in research relevant to Respiratory Medicine will be encouraged in collaboration with the Departments of Child Health and Human Nutrition of the University of Glasgow, members of the Scottish Paediatric Respiratory Interest Group (SPRING) and other relevant groups and individuals. Participation in audit projects both local and national that aim to assess and improve clinical practice will be expected. This will coincide with collection of data to enable measurement of clinical outcomes, comparison of outcomes with other centres and national figures, measurements of the respiratory workload and quantification of the resources within the respiratory service. Sessional time for participation in research will need to be negotiated with management. (vi) Promotion of the Respiratory MCN Development of the MCN with other health professionals in Yorkhill, the West of Scotland and Scottish tertiary centres should prove to be a particularly satisfying part of this new post. The aim is to provide a uniform standard of high quality respiratory care to all children within the West of Scotland. Liaison with clinicians and nurses in District General Hospitals in the West of Scotland to develop unified care pathways will be essential to achieve this aim. (vii) Continuing professional development Participation in continuing professional activities according to the requirements of the Royal College of Paediatrics and Child Health and the GMC is essential as will be appraisal and revalidation in due course. (viii) Training in Respiratory Medicine There will be provision within each of the above duties for training of specialist registrars undertaking a general paediatric or specialist respiratory training in order to enable Yorkhill to maintain recognition as a UK national training centre approved by the General Medical Council. Participation will also be expected in weekly educational meetings provided for trainees and other interested medical and paramedical professionals (e.g. a weekly respiratory meeting in addition to meetings currently dedicated to respiratory radiology and to general paediatrics). (ix) Undergraduate and Postgraduate teaching There are opportunities for participation in the undergraduate teaching programme of the Faculty of Medicine of the University of Glasgow will be a further feature.
