By signing this Order of Services, the Customer confirms having received and carefully read these documents, and agreeing thereto without reservations. The Customer undertakes to read the terms and conditions of all services that he/she activates throughout the term of the Subscriber Contract; the terms and conditions of the activated services shall form integral part of this Subscriber Contract as at the date of activation of the particular service. The Customer shall be provided with the electronic communications services defined by the Voice line Premium Terms and Conditions and terms and conditions applicable to the Additional Service, in the extent stipulated by the contracting parties. In the event the Customer refuses or repeatedly fails to collect the subsidized phone, notebook or accessory or other device through which he/she may use T-Mobile services (jointly referred to as the “Device”), T-Mobile shall store such a Device for the Customer for 12/24 months following the conclusion of the Subscriber Contract. Should the Customer fail to collect the Device within this time limit, the Customer hereby grants its consent to donating the Device to charity. This shall be without prejudice to the validity and effect of the Subscriber Contract. In the event that the Customer selects, at any time during the term of the Subscriber Contract, a tariff plan with business terms and conditions that stipulate automatic extension of the Subscriber Contract after the termination of the agreed fixed period of the Subscriber Contract, the Customer shall agree with the automatic extension of the Subscriber Contract. The automatic extension of the Subscriber Contract is regulated by the valid business terms and conditions of the selected tariff plan. T-Mobile and the Customer agree on the duration of Subscriber Contracts stipulated in the “Services Settings” which form integral part hereof. The Customer undertakes to observe the Minimum Monthly Payment in the amount and for the period specified in the Services Settings; such period shall start on the day of entering into the Subscriber Contract (and shall be extended by the period during which the provision of Services is suspended). The amount of the contractual penalty and details regarding the Minimum Monthly Payment are indicated in the Minimum Monthly Payment Terms and Conditions. The Customer undertakes to use the Voice line Premium service only at the installation address indicated in the Services Settings.
