The subject-matter of this Agreement is the transfer of the rights and obligations arising from the Subscriber Contract (the
“transfer”) between the parties to this Agreement. By signing this Agreement, the Customer assumes the rights and
obligations of the Subscriber arising from the Subscriber Contract. The content of the Subscriber Contract is formed by the
Subscriber Contract, the General Terms and Conditions of T-Mobile Czech Republic a.s. for Business Customers in force or,
as the case may be, the General Terms and Conditions of T-Mobile Czech Republic a.s. for Consumers in force, depending
on the position4) in which the Customer enters into this Agreement (the “General Terms and Conditions”), the Terms and
Conditions for Processing of Subscribers' Personal, Identification, Traffic and Location Data in force, the Price List of
Services in force, in the case the Customer also purchases a notebook, by the Special Terms and Conditions Governing the
Purchase of a Notebook and by the conditions of other services, particularly the terms and conditions relevant to the
selected tariff plan, to number porting and to any other services purchased by the Customer. By signing this Agreement, the
Customer confirms that he/she has received these documents, read them and agrees to them without reservations.
The Customer expressly confirms that he/she has read and agrees also to the General Terms and Conditions of T-Mobile
Czech Republic a.s. and the Terms and Conditions for Processing of Subscribers' Personal, Identification, Traffic and
Location Data effective from 26 January 2014, which will form an integral part of the Subscriber Contract from 26 January
2014.
The Customer will be provided with the basic and additional electronic communications services and other offered services
to the extent agreed by the parties. In the event that the Customer selects, at any time during the term of the Subscriber
Contract, a tariff plan with business terms and conditions that stipulate automatic extension of the Subscriber Contract after
the termination of the agreed fixed period of the Subscriber Contract, the Customer agrees with the automatic extension of
the Subscriber Contract. The automatic extension of the Subscriber Contract is governed by the valid business terms and

conditions of the selected tariff plan. The Customer undertakes to read the terms and conditions of all services that he/she
activates throughout the term of the Subscriber Contract; the terms and conditions of the activated services shall form
integral part of this Subscriber Contract upon the date of activation of the particular service.
Debts and receivables of the Original Subscriber related to the transferred Subscriber Contract are not transferred to the
Customer, nor are the settings of the services provided so far and customized offers that were provided to the Subscriber.
T-Mobile is entitled to interrupt the provision of services to the Subscriber during the technical implementation of the transfer
for the necessary period.
In the event of a breach of the obligations arising from the Subscriber Contract, the Customer and T-Mobile stipulate
contractual penalties defined in the General Terms and Conditions, the Price List of Services and other business terms and
conditions that form part of the Subscriber Contract.
Advice to SME customers: The SME customer is aware of its duty to pay the contractual penalty as set out in the Important
Customer Contract Terms and Conditions in the event that it fails to comply with the obligations arising from the Important
Customer Contract.
When selecting any of the Profi tariff plans, the Customer agrees that all SIM cards belonging to the same Customer
Contract as the phone number for which such a tariff plan was first selected by the Customer will be automatically
transferred to any of the Profi tariff plans according to the conversion table available at www.t-mobile.cz.
The Customer agrees that T-Mobile will process the Customer’s personal or identification data (“Data”) in compliance with
the Terms and Conditions for Processing of Subscribers' Personal, Identification, Traffic and Location Data and that T-Mobile
may provide information on the company registration number, business name and place of business for the purposes of
Article 5 of the Terms and Conditions for Processing of Subscribers' Personal, Identification, Traffic and Location Data.
The Customer agrees with the publication of Data in telephone directories and information services of the providers that
provide such services.
The Customer requests that the information that he/she does not wish to be contacted for marketing purposes be
published with the Data in telephone directory.
The person concerned is a politically exponsed person with respect to the section 4 article 5 of the Act No. 253/2008 Col.
In accordance with the Data Processing Terms and Conditions, the Customer agrees that business and marketing
information containing offers of T-Mobile’s products and services may be sent or provided to him/her

information containing offers of products and services of entities other than T-Mobile may be sent or provided to
him/her.
The Customer agrees that T-Mobile may obtain information on the Customer’s payment history necessary for the
purposes of reviewing the Customer’s request for entering into a Subscriber Contract from the database administered by
the SOLUS association (www.solus.cz), of which T-Mobile is a member.

The Subscriber and the Customer confirm that the Subscriber has communicated to the Customer all settings of services, including access passwords. T-Mobile
recommends that the Subscriber delete all saved/stored messages and immediately change all access passwords. T-Mobile shall not be liable, either to the
Subscriber or to the Customer, for any misuse of access to services caused as a result of failing to observe the aforementioned recommendations.
In the case that the transferred Subscriber Contract involves a phone number that has been included in the T-Mobile Private Network or, as the case may be, in the
T-Mobile Virtual Private Network (both hereinafter referred to as the “private network”), the original Subscriber and the Customer understand that the services of the
private network will be deactivated on the respective SIM card corresponding to the aforementioned phone number on the day on which the Contract is transferred;
this shall involve removal of the phone number from the private network.
