JOB DESCRIPTION Job Title: Social Worker, Transition and Liaison Team 0.5 wte secondment post up to March 2009 Salary/Grade: Social Worker Main Scale SCP 27 - 38 Location: St. Mary's Hospital, Kettering Responsible to: TLT Team Leader Purpose of Job: To support adults with complex developmental conditions namely Asperger`s Syndrome, Attention Deficit Hyperactivity Disorder (ADHD) and Tourette`s Syndrome, in particular those who are vulnerable and those with accommodation concerns. PRINCIPAL RESPONSIBILITIES 1. The post holder will be expected to work closely with all members of the multidisciplinary Transition and Liaison Team and contribute to service, practice and Clinical Governance developments, both within the Team and the wider network 2. The post holder will be required to work closely with other services and agencies to facilitate a smooth transition between the relevant services, e.g. Child & Adolescent Services to Adult Services. 3. Co-ordinate multi-disciplinary assessments particularly when the needs of service users are complex, causing conflicts between professionals, and requiring negotiation between services to ascertain appropriate responsibility of care. 4. To establish an effective communication network in order to liaise and negotiate with other professionals, service providers or agencies including statutory and voluntary sectors to ensure that service users and their carers will receive appropriate services to meet their needs. 5. Be responsible for managing and maintaining a small clinical caseload by developing and delivering specialist care packages which include coordinating multi-disciplinary therapeutic activities with other professionals and agencies. Management/Administrative/: The post holder will: 1. Participate in clinical supervision with the Team Leader of the Transition and Liaison Team. 2. Participate in other team based or Directorate-wide Clinical Governance initiatives. 3. Use available opportunities to develop special interests in accord with service need. 4. Actively participate in the promotion of the role of the specialist team. 5. To maintain the highest standards of clinical record keeping and report writing in accordance with professional codes of practice and Trust policies and procedures.
