General Scope of Work

PROPOSAL DATE: 07/28/2014

I.

MAIN POOL REVOVATIONS

A.

RESURFACE POOL (White coat)

1.

U.S. Aquatics (USA) shall drain pool

2.

USA shall cut beneath the tile line approximately ½” deep and remove the plaster about 2” down to better receive the new surface.

3.

USA shall chip around all of the plastic fittings (including lights and niches) in the same manner.

4.

Prior to white-coating, the pool surface shall be checked for hollow areas.

USA will remove up to 100 square feet. Any amount beyond 100 square feet will be at an additional cost to the Owner of $10.00 per square foot.

5.

USA shall pressure wash entire surface using a mild muriatic acid solution

6.

USA shall clean up, remove and dispose of all debris, to pre-construction condition.

7.

USA shall bond coat the cleaned surface

8.

Pool shall be plastered per standard plaster mixture and add mixes.

9.

Pool to be filled by Owner supplied water. Water to be balanced and maintained by Owner per Langlier Index.

10.

Pool should be brushed regularly for two weeks prior to vacuuming. Pool must not be drained prior to the end of 30 days for proper cure.

B.

REPLACE TILE

1.

U.S. Aquatics (USA) shall provide all labor and materials necessary to remove and replace water line tile.

2.

USA shall saw cut above and below the existing tile line

3.

USA shall utilize pneumatic tools to remove the existing tile

4.

The areas to receive the new tile are as follows: a.

Main pool perimeter – entire

5.

Features of the new tile line include: a.

Installation of Dal-Tile 2x2, frost-proof industrial tile. b.

Installation of rope anchors to replace existing

c.

Installation of depth tiles as are required by governing Health

Department

6.

USA shall install new tile with 2 part Portland mixture

7.

USA shall clean up, remove and dispose of all debris, to pre-construction condition.

C.

REPLACE COPING STONES

1.

U.S. Aquatics (USA) shall provide all labor and material to remove and replace coping stones.

2.

USA shall utilize air compressed jack hammers to remove stones.

3.

USA shall remove mud bed from the beam.

4.

The beam will be evaluated after the coping is removed and should there be any damage the owner will be notified and a proposal supplied.

5.

The coping stones shall have knock-out to accommodate depth and info tiles to match Dive Pool

6.

The areas to receive new stones are as follows: a.

Main pool perimeter – entire

7.

USA shall clean up, remove and dispose of all debris, to pre-construction condition.

D.

REPLACE CAULK

1.

U.S. Aquatics (USA) shall provide all labor to remove existing caulk from the specified areas.

2.

USA shall utilize a two part urethane based caulk compound.

3.

The areas to be caulked shall be as specified: a.

Main pool perimeter – entire

4.

USA shall clean up, remove and dispose of all debris, to pre-construction condition.

E.

ADA STEPS **

1.

USA shall provide all labor and material necessary to install (2) Two ADA rails approximately 20” wide, in the main pool.

2.

The steps will be tiled at step edge.

3.

The rise and tread shall conform to ADA standards.

4.

USA shall clean up, remove and dispose of all debris, to pre-construction condition.

**The Montgomery County Health Department no longer offers guidance with regulation. The design we are offering is based on current information provided by the current regulations. We suggest seeking further information from The Department of Justice if this moves forward.

F.

REPLACE TRUNKLINE

1.

U.S. Aquatics (USA) shall provide all labor and material necessary to replace the main pool trunk line.

2.

USA shall cut out and remove a 2.5’ concrete swath from the pump room to and around the main pool.

3.

Heavy machinery will be used to excavate and remove the existing pool lines if needed.

4.

USA shall excavate and remove plumbing lines to the main drain box (if found).

1.

USA shall chip out and cover existing floor returns.

2.

USA shall rock drill wall ports to accommodate new return fittings.

5.

USA shall install a new 4”, schedule 40 main drain line from the pump room to the main drain area and attach to (2) two 12x12 main drain boxes, located

3’ apart at the bottom of the Main Pool well.

6.

USA shall install a new 6”, schedule 40 skimmer line from the pump room to the new skimmers.

7.

USA shall install a new 2”, schedule 40 vacuum line from the pump room to the new vacuum ports.

8.

USA shall replumb all piping utilizing schedule 40 PVC pipes and fittings.

All connections will be solvent welded.

9.

USA shall cover pipes with sand, once covered, then clean fill dirt

10.

USA shall tamp every 6”

11.

USA shall install at the option of the OWNER one of the following materials to cover pipes:

Brick Pavers (or equivalent) – needs County Approval

1.

Additional charges will apply depending on material

2.

Concrete

12.

USA shall clean up, remove and dispose of all debris, to pre-construction condition.

G.

PROVIDE 2 SETS IN-WALL STEPS

1.

USA shall provide and install (2) two sets of in-wall steps and deck handrails.

2.

USA shall clean up, remove and dispose of all debris, to pre-construction condition.

H.

REMOVE 3 LIFEGUARD STANDS

1.

USA shall remove three (3) three lifeguard stands.

2.

USA shall provide (3) three portable stands (Astral)

I.

PROVIDE LIGHT RINGS

1.

USA shall provide new light rings for all existing light fixtures.

2.

USA shall install light rings to insure no “floating” fixtures

J.

PROVIDE AND INSTALL TILE ALLIGATOR

1.

USA shall install a 6’ x 8’ tile alligator on pool floor.

K.

INSTALL ADA LIFT

1.

U.S. Aquatics (USA) shall provide all labor and materials necessary to install a new ADA lift – Freedom Lift II.

2.

USA shall ensure the sleeve is appropriately bonded

3.

USA shall secure sleeve with concrete.

4.

USA shall clean up, remove and dispose of all debris, to pre-construction condition.

L.

INSTALL MERIMADE HAIR LINT STRAINER AND VALVE

1.

U.S. Aquatics (USA) shall provide all labor and materials necessary to install a new hair lint strainer (HLS) – 8”.

2.

USA shall replace faulty vacuum valve on suction manifold

3.

USA shall clean up, remove and dispose of all debris, to pre-construction condition.

M.

INSTALL NEW RACING LANES

1.

U.S. Aquatics (USA) shall provide all labor and materials necessary to remove existing racing lanes and targets tile.

2.

USA shall install 2 x 2 Dal-Tile (match perimeter) for lanes and targets

3.

USA shall use 2 part marble dust to 1 parts white Portland cement mixture

4.

USA shall clean up, remove and dispose of all debris, to pre-construction condition.

N.

REPLACE EXPANSION JOINTS

1.

U.S. Aquatics (USA) shall provide all labor and material necessary to remove tiled joints.

2.

USA shall install 2 x 2 Dal-Tile (white) for 4” on both side of joint

3.

USA shall use 2 part marble dust to 1 parts white Portland cement mixture

4.

USA shall caulk joint with Sika Flex white caulk

5.

USA shall clean up, remove and dispose of all debris, to pre-construction condition.

O.

REPAIR BEAM DAMAGE

1.

U.S. Aquatics (USA) shall provide all labor and material necessary to remove all damaged beam.

2.

USA shall inspect all rebar for damage. Damaged rebar shall be cut out

3.

USA shall drill holes in existing concrete to accommodate new steel and install and tie new steel.

4.

Industrial strength concrete shall be installed.

5.

USA shall clean up, remove and dispose of all debris, to pre-construction condition.

2.

1/3 is due at the beginning of work.

3.

Remaining balance is due upon completion of work.

4.

If total job cost is below $3,000.00, total balance will be due upon completion of work with no deposit required.

5.

Any payments received over 30 days past due are subject to a 2% interest payment per month. This charge will accrue on a monthly basis until all moneys due are paid in full.

6.

Customer agrees to pay all costs, including, but not limited to attorney’s fees, collection fees and any other costs associated with collection.

7.

The equipment/materials/accessories described in this agreement will not be the possession of the Owner until full payment is made to U.S. Aquatics. If payment is not received in accordance with this agreement, U.S. Aquatics may enter the facility without the Owners consent and repossess any pool equipment and accessories included in this agreement.

8.

In the event the OWNER breaches any of the terms of this contract, U.S.

Aquatics may stop work at their discretion. OWNER will forfeit all payments previously made to U.S. Aquatics and will be liable to U.S. Aquatics for damages cause by said breach and all costs incurred by U.S. Aquatics up to the time the work stopped.

9.

Work must begin at the discretion of U.S. Aquatics.

10.

Permits, inspections, surveys, drawings, fees and engineering are not included.

11.

Inspection of the property was done as a visual inspection only. Any necessary work found after the start of the job will be at an additional cost to the property owner and only performed upon the owner’s prior approval.

12.

U.S. Aquatics may stop any construction without penalty under the contract or incurring financial/legal responsibility if hazardous conditions including, but not limited to lead, asbestos, or medical waste are found. Owner agrees to remedy any hazardous conditions at their own expense if they are found.

13.

This agreement does not include the removal of any underground obstructions which would interfere with the normal progression of the work described herein. If they are found, the Owner must remedy the situation before U.S.

Aquatics can resume construction.

14.

U.S. Aquatics reserves the right to use subcontractors at their sole discretion for any of the work in this agreement.

15.

U.S. Aquatics shall not be held liable for mold, growth of mold, or effects of mold growth, directly or indirectly, at or within any area or building included as part of, or adjacent to, the OWNER’S property described, addressed, and/or named in this agreement.

16.

U.S. Aquatics cannot be held responsible for damage done to work by vandalism, pedestrian traffic or any Acts of God.

17.

U.S. Aquatics cannot be held responsible for damages of any kind attributed to unexpected underground conditions discovered during excavation, weather conditions, accidents, acts of civil or military authorities or any other condition, acts or circumstances beyond our control.

Information regarding the Whitecoat/Plaster:

a.

It is the Owner’s responsibility to shut off the water when the pool and spa are full. Do not stop the water until it reaches the middle of the tile line. b.

The warranty shall be rendered invalid and void if during the warranty period, the pool and or spa is drained by any persons other than USA or a person under the direct supervision of USA. c.

The warranty does not cover existing structural problems, soil problems, subsidence, ground movement (including earthquake), acts of war or civil unrest, or any problems that occur from an act of nature. d.

USA is not responsible for tiles falling off or coming loose during the undercut and chipping procedure. Tiles may be replaces by USA at

$15.00 per tile. e.

Plaster is composed of natural materials which have certain inherent characteristics. A certain amount of shading or color variation is a natural occurrence which should not be construed as a defect. Crazing

(check marks and hairline cracks) are common in pool plaster and may be more pronounced in colored plaster. Due to atmospheric conditions

(humidity, temperature, etc.) dark and or light streaks may appear in the plaster and may be more pronounced on a cloudy day, at night when the lights or on or in and indoor pool/spa. These conditions are beyond the control of the Contractor or it’s Sub Contractors’. f.

All warranties are void unless all payments are made to the Contractor.

19.

U.S. Aquatics reserves the right to substitute materials, equipment or methods of equal or better quality to that specified in this agreement without notification and no additional cost to the OWNER.

20.

Unless otherwise stated in this agreement, U.S. Aquatics will not be responsible for any seeding, sodding, or landscaping surrounding the facility.

U.S. Aquatics will clean up, remove and dispose of all debris from the work done in this agreement, however U.S. Aquatics will not be responsible for clean-up of dust created by winds or sandblasting.

21.

U.S. Aquatics shall reserve the right to void this proposal if not signed and returned by OWNER within forty-five (45) days after the PROPOSAL DATE.
