Diploma Correction Form
Class of 2014
Seniors are busy planning graduation. To ensure your name is correct on the
diploma, we have posted a tentative list of diploma names for students to verify
that their appears correctly i.e., middle name, spelling etc.
The list is located in front of the Main Office. Be sure that your student takes the
time to check that their name appears correctly (i.e., middle name, spelling etc.).
Note: Diplomas are legal documents - students are to use their legal name.
If a name correction is required, just complete the information below, and return it
to Mrs. Sears in the Main Office before the deadline.
