In the space below please provide a date when the activities were accomplished. Give one copy of this list to your building principal,
one copy to V. Walz at Community School, and send one copy to J. Hubbard at central office on or before January 15th.
The following activities have been completed as part of the mentor program during the months of July,
August, and September:

current evaluation report). Discuss casemanager responsibilities.
_____ Provide a list of modifications and accommodations to general education teachers. Due by 2nd
week of the new school year.
_____ Develop appropriate schedules coordinating with building principal and general educators. Send
schedule/s to DSE at the Community School for core data reporting. Due by 2 nd week of the new
school year. (Elementary).
_____ Compare student’s schedule to IEP Service Summary. Amend IEP’s as needed. (6 th-12th) To be
completed by the 2nd week of the new school year.
_____ Meet with special education team (psych examiner, SLP, OT, PT, other teachers) in the building to
discuss building specific procedures.
_____ Participate in NetChemia (web-based IEP system) training.
_____ Participate in Wilson Reading Training. (Elementary)
_____ Review/update caseload information on NetChemia and compare to caseload provided by special
education office. Verify name, dates, disability, and related services.
_____ Review Procedural Safeguards.
_____ Compete a special education calendar documenting IEP meeting and re-evaluation dates. Consider
the first IEP notification must be made 20 days before the IEP is due.
_____ Share effective practices regarding classroom procedures exclusive to special education (lesson
plans for a multi-aged classroom, objectives clearly stated, folders home, and data collection
procedures to document progress toward goal/s).

1600 Standards Manual).
_____ Network with mentor/psych examiner to review diagnostic reports in order to find the information
needed to write a compliant IEP and develop an understanding of the findings.
_____ Review effective practices for parent/teacher conferences, including documenting progress toward
IEP goals (progress reports).

15th.
