ABOUT MY PRACTICE!
Thank you for choosing me as your Primary Care Physician. I am a member of the South Shore Physician Hospital
Organization, Inc. (SSPHO). The SSPHO allows physicians, and the hospital, to work together to preserve and protect the value and quality of health care for the people who live and work in this community.
The SSPHO offers a comprehensive program of inpatient, outpatient, and physician services for your whole family. Our doctors represent 30 specialties and South Shore Hospital is the largest and most comprehensive facility on the South Shore.
In addition to providing comprehensive preventive healthcare services, my office provides extended office hours and/or 24-hour physician on-call coverage for your urgent care needs. Should this need arise, I feel strongly that calling this office will assist in providing you with the highest quality care in the most appropriate setting.
REFERRALS
Most of your medical needs can be managed in this office. Should you need a referral for specialty care, it is my policy to refer patients to specialists who are affiliated with the South Shore Hospital. This allows me to share information efficiently and coordinate the best treatment for you. If the expertise needed to handle a particular medical problem is not available from a
SSPHO specialist, I will select and refer you to another specialist. This seldom happens because the quality of our specialists and the scope of their care are superb and will satisfy most of your needs.
My office staff is well-versed in primary care services offered and the mechanism to effect proper use of the specialty care referral process. If you have any questions regarding our scope of care or the referral process, please do not hesitate to contact our Referral Coordinator or Office Manager.
ACUTE CARE NEEDS
If you are in need of hospitalization, I am affiliated with the South Shore Hospital, which is this region's leader in health care.
You will benefit from the expertise of a superior hospital whose mission is to provide high quality, efficient health care to the community.
If you are a patient at South Shore Hospital, I may arrange for your med ical care to be provided by a “hospitalists”. Hospitalists are physicians, usually board-certified, who specialize in the care of hospitalized patients. The hospitalists work in partnership with me to coordinate your care. Hospitalists also work closely with nurses, clinicians, and other physician specialists who might be involved in your care.
During your hospital stay, my team of Nurse Case Managers will assist with your discharge needs, ensuring your unique needs are met.
POST ACUTE CARE NEEDS
If you are in need of sub acute, rehabilitation or skilled nursing care, I recommend one of the following facilities that will assist with your transition home. A physician associated with the SSPHO will follow your care and communicate directly with me during your stay.

If you require home care services, the SSPHO's provider of choice for these services is the South Shore Visiting Nurse
Association (SSVNA). If these services are needed, I will work closely with your insurance company and health care team to coordinate this effort.
