A non-refundable fee of $90.00 must be made payable to St. Ann
Athletics along with the completed top portion of this form.
Applications received without payments will not be accepted.

Each sport requires a new deposit check of $35.00 which will be
held until all uniforms are collected at the end of the season.
Once the uniform is received, the deposit checks will be returned.
Please make a separate check payable to St. Ann Athletics for
your deposit.

Physical forms are required prior to evaluations.

Player evaluations will be held the week of Oct. 20th – Oct 23th.

Please do not request that your child be put on a team with a
particular coach, as we cannot honor requests of this nature.

I wanted to take a minute of your time to explain the selection process that your
child will go through as they enter 5th grade basketball. Unlike 1st – 4th grade
sports where each child’s name is drafted by random pick and assigned to a
team, 5th grade and up requires an evaluation and selection process.
For example: All 5th grade boys will be evaluated at the same time by coaches
and others who do not have children in this division. The coaches for this age
group will be allowed to be part of the process in evaluating and selecting
players. Each evaluator goes by the same criteria, and at the end of the
evaluations we will have our select team which consists of all 5th graders.
The remaining 5th graders who were not assigned to the all 5th grade team will
be assigned to a combo team which consists of 5/6th grade boys together. The
all 5th grade team will be in a league division that is competitive against other
PAA all 5th grade teams. The 5/6 combo teams play in a different division where
they will compete against only combo teams from other schools. All PAA schools
follow this process to establish the leagues and divisions each of the players and
teams are competing in during the season.
During the evaluations each child will be graded on how well they display
individual drills such as dribbling, shooting, ball handling, over-all form, passing,
and working together as a team.
Evaluators will have a score card with only a number on the card and not your
child’s name. As each player enters the gym they will be assigned a number to
wear on the back of their shirt. This way names are not available to those
evaluating, only a number. Once the evaluations are complete, teams will be
selected according to individual scores.
