Shire of Carnarvon CONDITIONS OF APPROVAL FOR EXCAVATIONS WITHIN ROAD RESERVES The following conditions for the opening up of roads should be read in conjunction with the Utility Providers Code of Practice for Western Australia (2002), Restoration and Reinstatement Specification for Local Governments in Western Australia 2002 and AUSSPEC #2 Specification 306. GENERAL CONDITIONS 1. No works shall commence on site until the form attached to these conditions has been filled out by the contractor and signed and approved by the Manager Design Services or his representative. 2. A reinstatement bond shall be payable before any works commence. This bond is payable and held until the excavations have been satisfactorily reinstated at which time the bond will be returned in full. The bonds payable are as follows – Excavation less than 20m Excavation greater than 20m $ 500 $ 1000 Bonds will be held for the duration of the works and will only be returned upon completion of the works and subsequent inspection and approval by the Director of Works & Services or his representative. 3. The contractor shall provide the Shire of Carnarvon with a traffic management plan, prepared to Australian Standards by an Advanced Worksite Traffic Management accredited person, clearly showing all signage and set up distances at the time of submitting this application. 4. Council by-laws, local laws and policies shall be complied with at all times. 5. All excavation work shall be supervised by a person experienced in the trade applicable. 6. It is the contractor’s responsibility to locate all services including stormwater prior to commencement of the excavation works. 7. The Shire’s Horticulture section shall be contacted prior to any excavation taking place within three (3) metres of the base of any trees within the road reserve (verge). 8. Notification shall be given to the Works & Services Department at least 5 working days prior to the commencement of work or recommencement of work after any prolonged cessation of the works. 9. Residents shall be advised of intended works at least 5 working days prior to the commencement of the works where they are deemed to be affected. 10. Cultivated lawns shall be removed for the full top width of the excavation by cutting with a sodding machine and the sods neatly stacked in a manner to ensure the maximum possibly preservation of the lawn. After completion such lawns shall be reinstated as near as practical to their original conditions. 11. In the event of any irregularities or emergencies occurring, the Shire’s Works & Services Department must be notified immediately of the problem(s) occurring and the intended remedial action. Any damages which may occur to Shire facilities, service authority property or private property during the course of the road excavation operations, either directly or indirectly or which may subsequently be evident from the operations thereof, shall be the sole responsibility of the applicant, who shall be responsible for he repair, replacement, legal claim liability or result which may arise from the road cutting. 12. 13. The attached application form, a traffic management plan and a site plan, indicating the exact location and dimensions of the road crossing, shall be submitted to the Director of Works & Services at least 5 working days prior to commencement of works. Duty of Care Anyone carrying out operations on a road open to traffic has a duty of care under common law to take all reasonable measures to prevent an accident or injury to persons carrying out the operations and also to members of the public lawfully using the road. ROAD/ROAD RESERVE TRENCH EXCAVATION Traffic Where works required control of traffic, appropriate traffic control devices to be put in place in accordance with AS 1742.3. The Contractor shall make adequate provision for all traffic using the road or footpath and shall erect such barriers, warning signs, etc. as are necessary to safeguard all traffic and accept all liability in connection with the works covered by this approval and will indemnify the Council against all actions, claim and demands made by any person or persons for injuries or damages suffered by him or them by reason of non compliance or non observance of the conditions and precautions in this Clause contained. Road warning signs, banners, lights and flagman shall be placed in accordance with the requirements of ‘Utility Providers Code of Practice for Western Australia (2002)’. Applications for permission to close roads shall be forwarded to the Council for prior approval thereof, at least 8 working days prior to the closing of any road to traffic. Such notification shall also be forwarded to the Police, WA Fire Brigades Board and ST John’s Ambulance Association and any other authorities as identified through the preparation of a traffic management plan. Road closures shall be kept to a minimum and every cut shall be backfilled and made trafficable immediately pipe laying has been completed. All roads will have at least half of the carriage way open to traffic at all times, ie. only half the road may be excavated at one time unless alternative arrangements are made with the Director of Works & Services. Trenching The road/verge surface (bitumen, asphalt or concrete), shall be cut with an approved saw prior to the commencement of excavation works. Stockpiling of any excavated materials other than immediately adjacent to the excavation shall be on approved sites only. Excavations that are the responsibility of Main Roads WA (North West Coastal Highway and Exmouth/Minilya Road) require the contractor to notify Main Roads WA in Carnarvon at least 15 working days prior to commencement of works. RESTORATION AND REINSTATEMENT SPECIFICATION FOR BACKFILL OF TRENCHES Reinstatement of the surface of roads, vehicular crossovers and footpaths shall be undertaken by contractor, at the contractor’s expense in accordance with the following documentation AUS-SPEC #2 Specification 306 Restoration and Reinstatement Specification for Local Government in WA 2002 The contractor if unable to reinstate the trench is to issue a Reinstatement Order to the Shire immediately after work is completed, if applicable. Competent contractors may reinstate road crossings within subdivisional releases, subject to the prior approval of the Director of Works & Services. The work is to be covered in the maintenance period of the subdivisional release. Materials Only approved material may be used as backfill material for road crossings for the full depth except for the upper 300mm which shall consist of gravel or crushed rock road base, evenly graded with a maximum aggregate site of 19mm. Materials for backfilling trenches across driveways and compaction specifications shall be the same as for road crossings, with the minimum depth of gravel being 200mm. Materials for backfilling trenches across footpaths shall be clean sand or other material approved by the Director of Works & Services to be brought up to the full depth of the trench. No roadbase need be used. On kerbed roads the gravel must be continued out past the edge of bitumen by 0.5m on either side and on unkerbed and unsealed roads the gravel must be extended to the full formation width. For reinstatement works using concrete the following shall apply – 100mm deep concrete construction for residential areas including footpaths and driveways Concrete mix 20 Mpa with 75mm slump Excavated site to be inspected by Shire staff before concrete is poured Verge Compaction/Reinstatement Before the verge backfill of the trench is commenced, all manholes and spaces around the utility installed shall be carefully compacted with hand rammers. The minimum depth of initial hand compaction above the crown of the pipe shall be no more than 150mm. Compaction of the remaining backfill shall be in 300mm layers. Maximum compaction shall be achieved by mechanical means to a density of not less than 95% of the maximum dry density when tested in accordance with AS1289.5.4.1. The crushed rock roadbase pavement shall be compacted to 98% of the maximum dry density when tested in accordance with AS1289.5.4.1 Verges shall be backfilled to their original level and compacted equivalent to surrounding ground or as required by the Director of Works & Services. Verges to e left in a clean and tidy condition free of debris. All verge backfilling will be covered by a 12 month maintenance period that commences from the date that the work is completed.
