PETITION
(to be typed on official letterhead of the public agency or river support group)

conservation plan) in accordance with the Pennsylvania Rivers Conservation Program and hereby petitions the
Pennsylvania Department of Conservation and Natural Resources to include the following watershed, rivers or river
segments on the Pennsylvania Rivers Conservation Registry.

List the watershed, rivers and river segments that you are requesting be placed on the registry. (Please use physical
landmarks to define these segments. Also attach maps or reference maps in the plan that define watershed /river
corridor boundaries and termini. Use additional sheets if necessary.)
The petition is supported by the following:
1.
Copies of the Final River Conservation Plan and executive summaries.
2.
Copies of notices, as advertised in local newspapers, of all public meetings and workshops.
3.
Copies of minutes from public meetings or workshops.
4.
Copy of a letter transmitting the Final Plan or Executive Summary Report to a municipality for review.
5.
Copy of municipal and watershed resolution supporting the river conservation plan.
Letters of support from fifty (50%) percent of the affected municipalities within the study area.
(Applies only to river conservation plans not originally funded through the Pennsylvania Rivers
Conservation Grants Program. Please identify these municipalities and append letters of support.)
