General Summary of Duties: Support mission and goal achievement, organizational quality and patient-centered care.
Incorporate core organizational values of quality, respect, partnership, compassion and integrity in all activities and
decisions. Provide behavioral health services and interventions to targeted clients and consultation to medical providers
and related care team members utilizing an Integrated Behavioral Health (IBH) model. Provide more traditional
counseling including assessment and treatment planning within assigned School-Based Health Center (SBHC) setting.
Provide coordinated team care and case management services for medically complex patients with co- morbidities,
address issues of substance abuse with teen population, and encourage smoking cessation. Provide patient care and
education focused on patient engagement and behavioral change, treatment compliance and health risk factor reduction.
Support patients in management of their chronic diseases. Support care team implementation of evidence based clinical
best practices. Ensure the efficient and effective flow of patients during scheduled clinics. Comply with organizational
policies, procedures, standards of care and related laws and regulations including HIPAA and OSHA regulations. Protect
the dignity, privacy and confidentiality of patients and their families, as well as co-workers and others.
Identify and assess behavioral health issues among students and refer students to appropriate resources as needed. Work
co-operatively with staff and parents to help remove barriers to good health and health care access with emphasis on the
at-risk and special needs students.
Provide excellent customer service to all internal and external customers, which includes team members, patients,
volunteers, students, visitors, and vendors, by consistently exceeding the customer’s expectations.
SUPERVISON EXERCISED: Supervision is generally not a responsibility of this position;
SUPERVISON RECEIVED: Works under the direct supervision of the Integrated Health Program Manager
ESSENTIAL FUNCTIONS: Behavioral Health Specialist
1. Provide brief, solution focused behavioral health care with targeted populations including assessments and
intervention geared toward positive behavioral change
2. Provide clinical consultation to medical providers and related team members in areas of behavioral health, case
management, motivational interviewing, A &D and related issues
3. Co-manage patient’s behavioral health care needs with consistent, expedited feedback to referring provider
4. Provide on-site crisis triage and intervention as needed
5. Support enrollment in organizations Quality of Life Program for chronic pain management through preenrollment interviews and participation in group and individual C of L meetings
6. Maintain proper documentation in medical records according to clinical guidelines and operational procedures
7. Complete and submit individual encounter forms
6. Collaborate with relevant community organizations to establish resource networks
7. Educate CHC staff, patients and community members regarding the role of the Behavioral Health Specialist as a
member of the care team
8. Participate in quality improvement initiatives and ongoing continuous improvement efforts
9. Assist in orientation and training of new staff and/or student externs
10. Comply with organizational policies, procedures, standards of care and related laws and regulations including
HIPAA and OSHA regulations
11. Other duties as assigned by supervisor
ESSENTIAL FUNCTIONS: Exceptional Needs Case Management
1. Provide case management services mental health patients in collaboration with the PCP care team
2. Plan with the patient, family, and care team for care that is feasible and within the physical, financial and
emotional resources of the individual
3. Develop, implement, evaluate and communicate to the patient/family and care team members plans for managing
the patient’s care needs
4. Facilitate referrals to community resources needed to manage the patient’s care
5. Facilitate communication between patients and their care team including community stakeholders when
indicated
6. Coordinate responses to patient needs and suggest solutions to patient care problems
7. Interpret to the patient and family diagnoses and treatment plans at the request of the patient’s Primary Care
Provider
8. Assist patients/families in setting self-management goals
9. Provide or assist patients to secure disease specific education as indicated
10. Maintain accurate and legible records of interventions
11. Perform related duties necessary to ensure quality patient care and provider productivity

EDUCATION: Graduate degree in social work or related field, LCSW Required
EXPERIENCE: Minimum two years experience in the field of Behavioral Health
QUALIFICATIONS:
 Current Oregon license as a Clinical Social Worker
 Current CPR certification
 Integrated behavioral health philosophy
 Ability to communicate effectively with a wide range of professionals, audiences, families and staff both verbally and
in writing
 Experience working with an interdisciplinary team
 Experience working with the low income population
 Conversational English/Spanish preferred
 Demonstrated teaching ability and experience
 Non-judgmental attitude
 Ability to exercise independent judgment
 Ability to handle confidential material
 Ability to communicate verbally and in writing
 Ability to work as a member of a team
 Ability to report for work as scheduled





Physical Demands: Lifting, bending, reaching, pushing, carrying, sitting, vision and hearing corrected in normal
range
Hazardous Conditions: Exposure to infectious diseases; potential exposure to physical violence
Work Condition: 95% inside; close contact with sick people
Machines, equipment, tools and supplies used: computer, copier, calculator, multi-line telephone system
Multiple Duties: Must be able to work under conditions of frequent interruption and be able to stay on task
