Registration /Agreement Form:
A DEPOSIT OF 50% of the selected scout package is required 4-8 weeks prior to the event along with
the completed registration form and any additional information.
The remaining of the balance MUST be paid the day of the event. In the case of sickness or other
cancellation the week of the event, another date may be selected. Deposits will be held & rolled over
into the remaining balance. Deposits will not be refunded. CHLAC ACCEPTS ONLY CASH, CHECKS
OR MONEY ORDERS. All checks & money orders are to be written out to Kelly Gillow.
There will be a $50 fee for any / all bounced checks. Fee will be paid directly to Kelly Gillow.
All people at the event MUST leave by the end of the event time selected, due to other possible parties
awaiting the party spot in the studio, unless additional party time is available.
I understand that Country Home Learning & Art Center/ Country Clay Studio and Pocono Party
Perfections will not be held responsible in any way for any/all allergic reactions or any all/medical
conditions that may occur with any food consumption or with the usage of any/all art or clay
products. We will also not be held responsible in any way in the case of any injuries that may occur
during any & all studio events.
C.H.L.A.C. / Country Clay Studio ART & CLAY FOR SCOUTS CANCELLATION POLICY:
I also understand that in the case of an event cancelation, all deposits are non-refundable. CHLAC/
Country Clay Studio/ Pocono Party Perfections will NOT refund the required 50% deposit paid 4-8
weeks prior to the party date. The remaining of the party balance will be due on the rescheduled event
date selected.
