Instructions for Completing the On-Line Registration Form
1. “Save As” with your LAST NAME as the document title.
2. You can complete the registration form on your computer by tabbing through the fields.
a. Fill in your name and any advanced degrees
b. Complete your address, along with position title, department, and organization. This makes up our
participant listing that will be distributed at the workshop.
3. Please choose an area of interest. This is important for determining the small group composition.
4. Figure out if you qualify for any discounts. We can only offer a limited number of discounts, so please call to
make sure the discount is still available.
5. Total up the workshop fees, and select a method of payment. Please make checks payable to the University of
Colorado Denver, EBHC Workshop. Our tax ID number (FEIN) is 84-6000555. If you wish to pay by Visa or
Mastercard, please contact Jennifer McIntyre Stachelski for a separate form to process your credit card. There
will be an additional charge to cover the credit card processing fees.
6. Fill out the questionnaire on page 3 of the registration form to help us assess your level of experience with
searching and with utilizing evidence-based medicine techniques in your practice setting. Please be very
realistic, even a bit cautious, about assessing your abilities.
7. Save the form with all of your information and either:
a. Attach the document to an email to Jennifer.Mcintyre@ucdenver.edu (preferred method)
b. Print it out and fax it to 303-724-4476. Please be sure to include a cover sheet addressed to Jennifer
McIntyre-Stachelski.
