WELCOME TO TAHLEQUAH PUBLIC SCHOOLS!
Enrollment considered complete when all forms and documents listed below
have been submitted to the Enrollment Office.
*Only parents(s) or legal guardian(s) which have required guardianship
documentation may enroll students.
* Only legal name on birth certificate used to enroll students or legal documentation of change(s).
REQUIRED PROOF OF RESIDENCE OF PARENT/GUARDIAN
1) Cherokee County Assessor’s office printout of address OR filed Homestead Exemption form
AND
2) Provide a copy of one of the following: Previous month’s Utility bill, Previous month’s phone bill,
CDIB picture ID, Current lease agreement, or Valid Oklahoma Driver’s license with
Guardians
name and physical address.
House Bill 557, effective July 1, 1997, implements a $500 penalty for those providing
false residence information to schools.

State Certified Birth Certificate – PARENT BRINGS TO ENROLLMENT
Social Security Card (if applicable) – PARENT BRINGS TO ENROLLMENT
Sooner Card/Medicaid (if applicable) – PARENT BRINGS TO ENROLLMENT
CDIB (if applicable) – PARENT BRINGS TO ENROLLMENT
Open Transfer Form (if applicable enrollment office will provide)
Student Enrollment Information – white form attached
Home Language Survey Form – white form attached
Title 1 Pre-Information Form – yellow form attached
