1. Proof of residency –
For parents who own their residence: a current residential property tax statement or HUD Settlement statement for
recent purchases.
For parents who are renting their residence: both a lease or rental agreement and the district affidavit signed by the
property owner.
For parents who are occupying another person’s home: a district affidavit signed by the parent/guardian and a
district affidavit signed by the property owner.

3. Health forms –
DHR Form 3231 Certificate of Immunization
DHR Form 3300 Certificate of Ear, Eye and Dental Exam
4. Withdrawal form from previous school attended.
5. Official Transcript –
Students shall present an official transcript of work or credit at the time of entrance. If a transcript is not presented,
the student shall be enrolled on a tentative basis from information available at the time of enrollment. Students
coming from accredited private schools, home school programs, other countries, or states are to be accepted
provisionally in the grade to which they indicate membership.
6. Discipline –
Students are required to provide documentation that all discipline issues from previous school attended have been
cleared.
7. Social Security Card –
A copy of the original child’s social security card shall be provided.
8. Verification of Parent/Guardian –
All parents/guardians enrolling students will be asked to provide a picture ID at the time of enrollment. The name
on the ID should match the name on the documentation being provided for enrollment.
Items missing (circled) are required before the student may attend RRHS. Thirty days are allowed to
obtain health forms and official transcripts for public school system transfer students only. Thirty day
grace period (for health forms and transcript only) will expire on: _________________
