Dean Health Plan www.deancare.com/providerportal Prevea360 www.prevea360.com/providerportal Children’s Community Health Plan (CCHP) www.childrenschp.com/providerportal Login Enroll New Customer Enroll New Customer If you are new to the Dean Health Plan Provider Portal select Enroll New Customer located at the bottom left corner of the login page. Then complete the Provider SelfEnrollment section. The first person who enrolls as a new customer becomes the Provider Portal Super User/Site Administrator for the site. The Provider Portal Super User/Site Administrator acts as a security officer. Provider Portal Super Users/Site Administrators have the ability to create new users, edit user information, reset passwords and activate or deactivate users through the Manager Users link. Section 2 PROVIDER SELF-ENROLLMENT Provider Self –Enrollment Pre-Registration Organization Primary Contact Provider Info Pre-Registration After selecting Enroll New Customer, the Pre-Registration section will appear. The first step in the pre-registration for self-enrollment process is to complete the following required fields. Email Address Organization Tax ID Security Word After you complete the pre-registration fields click theSubmit Please Note: A valid email address, tax ID and appropriate security word are required. You will receive an email from officenotify@emdeon.com indicating receipt. Please note: If you do not receive an email, there is a possibility it went to your junk email. The email will be sent to the email address entered and will contain a direct link to complete the enrollment. Thank you for your enrollment request. Please follow the link below to complete your enrollment https://officemail.emdeon.com/SelfEnrollment/Enrollment.aspx?Guid=de4d62f9f875-4877-91de-63b1324f01bb If you have not resumed your enrollment within 3 days, your request will be deleted. You will need to start the process from the beginning if you still wish to obtain an account after your request is deleted. The user will be directed to complete the self-enrollment by entering detailed information about their organization, primary contacts and provider details. Refer to next step. Organization Atypical Provider Disregard-Doesn’t apply Add the organization information for your Primary office, clinic, agent or hospital. The search criteria steps marked with a red asterisk (*) are required fields. When complete click Next Primary Contact The primary user will be considered the designated contact and serve as the site administrator. The search criteria steps marked with a red asterisk (*) are required fields. When complete click Next Provider Information Payer Assigned ID is not needed. The search criteria steps marked with a red asterisk (*) are required fields. Information from the previous page can be imported by clicking onto Copy From Organization. Please note: The Payer Assigned ID is not needed. Pick the specialty that best applies to you as a Provider Portal Super User/Admin user. The Add Provider button will complete the setup for one provider and can be reset to allow for multiple entries. Repeat the process by entering the required fields with the new provider and click the Add Provider button. Complete the setup by selecting the Finish button. Self-Enrollment Complete Once the setup is complete, the user will be supplied with a popup that details the user Id, password and login page link. Enrollment Message Congratulations! Your user account has been created and is ready for use. To access your account please use User ID: primary/user Password: msm Login Page: https://office.emdeon.com/vendorfiles/Dean.htm. PLEASE MAKE A NOTE OF THIS PASSWORD AS IT WILL NOT BE PROVIDED TO YOU AGAIN. If you lose your password, select the I Forgot My Password link from the Login Page to reset it. OK An email confirmation will then be sent to the primary contact (Site Administrator) with the user Id and a link to the login page. Your user account has been created and is ready for use. To access your account please use User ID: primary user Login page: https://office.emdeon.com/vendorfiles/Dean.htm. If you lose your password, select the I Fort My Password link from the Login Page to reset it. The new user can now login.
