General
- Place an X next to each statement that applies to your chapter and attach supporting
documentation.
- Directly below each statement, the required supporting documentation is indicated.
- All sections in the Chapter Report refer to chapter accomplishments between January and
November 2015 (the 2015 calendar year, including the Spring 2015, Summer 2015, and
Fall 2016 semesters).
- If a statement does not apply to your chapter, write “N/A” in place of an X and provide a
brief explanation as to why it is not applicable.

Assembling the application
- Indicate which statements a document supports. For example, a single photograph could
serve as documentation for Community Service statements #5, #6, #7, and #11.
- If you attach documentation in hard copy, please organize it in folders or a binder and
denote sections with tabs or dividers that correspond to award categories, e.g.
Recruitment Practices, Philanthropy
- If you attach documentation electronically, please use a similar method, organizing and
labeling documents according to category.
Tips
-
Some statements require specific types of documentation as noted. If this specific
documentation is not provided, your application will not receive credit for the response.
Highlight significant information in a piece of documentation that includes a lot of text.
Keep in mind that the judges will consider events and accomplishments that occurred
only between January and November 2015 (Spring 2015, Summer 2015, and Fall 2015
semesters).

Community Service
The Community Service Award recognizes a Greek chapter that has made outstanding
contributions in the area of community service. Community service is defined by hours spent
volunteering in and contributing to the local community.

Philanthropy
The Philanthropy Award recognizes a Greek chapter that has made outstanding contributions in
the area of philanthropy. Philanthropy is defined by the programming implemented to support
charitable organizations.
1. ______ The chapter elects or appoints an officer whose primary responsibility is planning,
implementing, and coordinating the chapter’s philanthropy.

Recruitment Practices
The Recruitment Practices Award recognizes a Greek chapter that has adhered to the values of
community, fairness, and responsibility in its recruitment/intake practices.

coordinate the chapter’s membership recruitment program.

New Member Programming
This award recognizes chapters that have developed and implemented a comprehensive New
Member Program that teaches, reflects, and demonstrates the principles and ideals that Greek
organizations were founded upon, including academic excellence, community service, personal
development, citizenship, and friendship.

planning and implementation of educational programming for chapter’s new members.

Risk Management
The Risk Management Award recognizes a Greek chapter that has shown excellence in
promoting and ensuring its members’ safety and contributed to the safety and welfare of the
larger Greek Community.
1. ______ The chapter elects or appoints an officer whose primary responsibility is to
educate members about and enforce chapter policies regarding risk management.
Documentation:

- Officer goals and objectives
2. ______ The chapter has up-to-date written policies regarding risk reduction and
management that include established methods of enforcement and that prohibit the use of
illegal substances.

- A copy of the policies
- Evidence of their adoption and/or revision (e.g. meeting minutes that include
the passage of bylaws, evidence of revisions to constitution or bylaws)
3. ______ The chapter has provided its assigned Greek Intern with its up-to-date written
Risk Management policies with the attached officer signature page for both the Spring
2014 and Fall 2014 semesters.

This award recognizes chapter excellence in alumni/alumnae relations, especially a chapter’s
consistent effort and programming to remain connected to members with alumni/alumnae status.

This award recognizes a chapter that has implemented comprehensive leadership development
and educational opportunities for its members. Additionally, this chapter promotes leadership of
its members throughout the University and greater Madison community.

Cross-Council Interaction
This award recognizes a chapter that is dedicated to interaction with chapters in another

interact with members of the entire Greek Community. This chapter truly understands what it
means to be Greek.

and implementation of interaction between its chapter and chapters of other councils
within the Greek Community.
