Parents will attend, with a completed application (for each child & each class) and cash or check made payable to Davis PTA. Students may be registered by a designated adult other than the parents if a parent is unable to attend. However, that designated adult may only register the children of one family. Your child's application will be reviewed in your presence to check for completion and accuracy. Applications and checks must be received on the evening of Wednesday, January 13, 2016. No exceptions will be made for late applications except for classes which do not meet capacity in which case applications may be accepted through Friday, January, 15, 2016. Communication will be go out if space is available and if additional applications will be accepted after registration night. Please fill out the attached application and bus form completely and carefully. An application must be filled out for each child AND each class. Forms with missing information will not be processed and will be returned to you. Applications must be accompanied with full payment of all course fees and received by the above deadline to be considered. All classes will be filled on a first come basis. If any classes are not filled on the original registration night, a second registration may be scheduled. The Spring session will begin on Tuesday, March 1, 2016. A l l classes for the Spring session will run for 10 weeks unless otherwise indicated on the calendar. Please refer to the calendar on the last page of the program to determine the precise schedule for each class. Classes begin at 3:15pm and end at 4:30pm. Any class cancelled due to foul weather, instructor absence, or any other emergency, will be rescheduled. All children will receive a calendar on the first day of class. Please read the calendar carefully and note the class schedule. The emergency contact information provided on the application must be different than the parents’ contact information provided. A separate application is required for each child and each class registering for After School classes. In addition, a separate check must be written for each class for which a child is registering. All checks must be made payable to Davis PTA After School Program. All applications must be received at registration on Wednesday, January 13, 2016, at Davis School in the cafeteria. NO applications will be accepted at any other time. Again, please be aware of the firm January 13 deadline. To ensure that 1) the committee meets all of its deadlines (e.g., late bus scheduling, instructor confirmations, etc.) and 2) the program runs smoothly, it is essential that we receive your applications and payments on January 13, 2016. POLICY THE DAVIS AFTER SCHOOL PROGRAM This program provides opportunity for participation in a variety of activities in sports, crafts, and performing arts. Instructors with educational experience are recruited from the Davis staff and local organizations. We are welcoming back our Director, Vincent Caporaso. He has been a Special Ed teacher at Davis for many years and has administrative experience. Mr. Caporaso will help ensure supervision and safety of your children at all times. Vincent Caporaso can be reached at 914-576-4420 during program hours. Parents should be aware that the After School Program takes place after school hours and that there is no nurse on school premises while the After School Program is in session. PROCEDURES Upon dismissal from school at 3:15pm, all children in the After School Program must report to the Auditorium where their activity instructors will meet them. The Director, Vincent Caporaso, will escort kindergartners, first graders, and students in special education to the auditorium. Parents should remind their child when they have an After School class scheduled for that day. If any parent has reservations about their child remembering to attend the After School Program, they should send a note to their child’s homeroom teacher. All children will return to the auditorium at 4:30. Parents may enter through the front entrance to pick up their child. LATE BUS Late bus service will be available at 4:30 for all children who normally take the bus. The After School Program is organized by parent volunteers who have no connection to the transportation office or the bus company. The After School Director, Vincent Caporaso, and your child’s instructor will be available to ensure that students get on their buses safely. If you have a problem with the bus, please call the Transportation Office at 576-4230 or First Bus Co. at 668-2700 or 668-2800. Please return the completed bus form WITH the After School Program Application. PLEASE DO NOT SEND THE BUS APPLICATION TO CITY HALL. PICK-UP AND LATE PICK-UP Parents must enter the school through the entrance at the front of the school. All other doors will be locked. All children will be dismissed from the auditorium at 4:30 (or walked to the Boys and Girls Club if enrolled on those days). It is imperative that children be picked up in the auditorium, so that the Director can keep track of the dismissal of all children. It is essential that the parents of children who do not ride the bus make arrangements to have their children picked up by 4:30pm. Children who are consistently picked up late may be asked to withdraw from the program and will not be granted a refund. To ensure the safety of our children, parents must inform the Director, in writing, of any alternative arrangements that are made for pick-up on a particular day (e.g. carpool changes and play dates). In case of an emergency, no child will be left unsupervised. However, a $15 fee, payable to the Director, will be charged to parents who do not pick up their children by 4:45pm. In the rare event of an emergency that requires a parent to pick up their child late, the parent must notify the Director at 576-4420 WEATHER EMERGENCIES The After School Program will follow the district lead when canceling classes due to inclement weather. Listen to WVOX (1460 AM) for this information. In the event that a program must be cancelled due to weather or emergency situations, parents will be notified by the robo call system. The After School Committee will no longer make individual calls. Parents please be aware you will only receive the robo call. Children will be sent home by their usual means of transportation at the time of the school's dismissal. A NOTICE OF NON-DISCRIMINATORY POLICY AS TO STUDENTS: The Davis After School Program admits students of any race, religion, color, national and ethnic origin or handicapping condition to all rights, privileges and activities generally accorded and made available to students. At the discretion of the instructor, a parent may be asked to withdraw a child from the program due to significant behavior problems. Learn the fundamentals through a mix of tennis instruction, rules & etiquette lessons, educational components, character development lessons and physical activity as students advance through the five-level program. Certified instructors have a passion for working with children and are trained to help student athletes develop a strong foundation of skills and knowledge as well as a passion for the sport. Join us for Fast Break Basketball where we’ll “cross-over” between GAME PLAY and skills stations. The games within the game are where we expose players to HOOPS skills like dribbling, passing, shooting, rebounding, and defense. All of the planes we are building in this class will be inspired by what aircraft of the future might be like! We’ll get started by building our Unlimited, a special rubber band powered helicopter that you will get to design yourself! You’ll really be able to open up your mind and dream up a cool helicopter from the future! Next we’ll build the amazing Seagull, a lightweight electric plane that is designed to fly as far as a football field! The Airhawk is a super durable free flight glider made out of balsa wood and foam that will be built from scratch. This plane’s design, when first introduced, was considered the “future of planes.” In addition to building and flying cool model airplanes, we’ll also learn about the history of flight, and what the future might hold.
