2015-2016 UNUSUAL ENROLLMENT
HISTORY WORKSHEET
The U.S. Department of Education has selected your file for review due to your unusual enrollment history, which is
defined as having attended and received Federal Pell grant funds from multiple colleges/universities during the review
period of the 2011-12, 2012-13, 2013-14, and 2014-15 academic years. Wor-Wic Community College is required to
review your enrollment history and determine whether or not you are enrolling only long enough to receive federal
student aid. In the process of reviewing your enrollment history, the Financial Aid Office will utilize federal databases for
a complete history of the Federal Pell grants you have received.
The following information must be received and reviewed before your 2015-16 financial aid can be awarded.

2011-12, 2012-13, 2013-14, and 2014-15 academic years.
You are required to submit an official transcript from each institution to the Wor-Wic Community College
Registrar’s Office for evaluation, regardless of whether or not you earned credit at that institution. If you
did not earn credit at an institution where you received a Pell grant, you must also attach a written
explanation for your failure to earn credit, as well as third-party documentation of any circumstances that
caused you not to earn credit (e.g., medical bills, hospitalization records, military transfer orders).

ADDITIONAL INFORMATION REGARDING
UNUSUAL ENROLLMENT HISTORY
The U.S. Department of Education has established regulations to prevent fraud and
abuse in the Federal Pell Grant program by identifying students with unusual enrollment
histories.
What this means for you:
 If you received the Federal Pell grant at multiple institutions in recent academic years (2011-12,
2012-13, 2013-14, and 2014-15), your 2015-16 Free Application for Federal Student Aid (FAFSA)
will be flagged (see next bullet for flag indicators) for unusual enrollment history (UEH).
 The flags “2” and “3” will be indicated on your Student Aid Report (SAR) and our Financial Aid
Office will be required to review your enrollment history to determine whether or not you are
enrolling only long enough to receive refunds of federal student aid.
 If you received the Federal Pell grant and credits were not earned at each institution (with
passing grades of A-D), you may be determined to be ineligible for federal financial aid.
 The Financial Aid Office will determine if you had a legitimate reason for having unusual
enrollment at multiple institutions.
 You will be asked to complete an Unusual Enrollment History worksheet (see reverse side of this
document).
 You will be asked to provide valid third-party documentation explain the reason(s) for your
unusual enrollment history.
Please note that unusual enrollment history must be resolved
before you can receive federal financial aid at Wor-Wic Community College.
UEH Checklist for Approval of Continued Eligibility





Complete and submit the UEH worksheet.
Submit an official transcript from each
institution to the Wor-Wic Community
College Registrar’s Office for evaluation.
If you did not earn credit, submit a written
statement and third-party documentation of
any circumstances that caused you to not
earn credit.
Meet with your academic advisor to complete
an Academic Plan for Unusual Enrollment
History.
If your continued eligibility is approved, you
must continue to meet all other eligibility
requirements for financial aid.
Regaining Eligibility if Continued Eligibility is Denied

If your continued eligibility is denied, you
must regain your eligibility by earning a
cumulative GPA of at least 2.0.

The courses in which you enroll to regain
eligibility must be paid for using your own
resources.
